Goodpasture's Syndrome Superimposed on Membranous Nephropathy. A Case Report

Abstract: We describe a patient with idiopathic membranous glomerulopathy who developed acute deterioration in renal function; this was associated with hemoptysis, severe hypertension, and anti-glomerular basement membrane (anti-GBM) antibody in the serum. Despite aggressive therapy with plasmapheresis, cyclophosphamide and prednisone, the patient progressed to end-stage renal failure and is on maintenance hemodialysis.

“…5,13,19 In approximately two thirds of the two dozen or so published reports, there was evidence of evolution from preexisting nephrotic syndrome, and in about half, a previous kidney biopsy showed typical MN. Progression to end-stage renal disease (ESRD) has usually been rapid, but the diagnosis has rarely been made at an early enough stage to expect intensive treatment to be successful.…”

“…Anti-GBM GN preceding MN, though rare, has been described in adults [1][2][3], and a single case in a 17-year-old smoker has been reported [4]. To the best of our knowledge, this is the second and youngest reported case in a child.…”

“…The likely pathological mechanism underlying the course of immune complex-glomerulonephritis with exposure of GBM antigens might subsequently lead to the production of anti-GBM antibodies, as supported by animal model studies [16]. Moreover, several case reports describe membranous nephropathy evolved to anti-GBM disease [17,18]. On the other hand, progression of anti-GBM disease to membranous nephropathy has been documented in several reports [19][20][21].…”
