ObjectivesThis study assessed the feasibility and preliminary efficacy of a 2-hour compression-only cardiopulmonary resuscitation and automated external defibrillator (CO-CPRAED) course in secondary school students.DesignProspective pre-post feasibility study.Setting and participants128 students (12–15 years old) without prior basic life support (BLS) training at four secondary schools in Hong Kong. All students were followed up at 3 months after training.InterventionsEmergency medicine-trained nurse and physicians taught the 2-hour CO-CPRAED course using the American Heart Association ‘CPR in School Training Kit’ programme. Students were trained in groups up to 40 students/session, with an instructor to student ratio not exceeding 1:10. To practise hands-on compressions, the manikin to student ratio was 1:1. For a simulated cardiac arrest, the manikin and AED to student ratio was 1:10.Primary and secondary outcomesCPR and AED knowledge, attitude statements towards bystander CPR and AED, quality of BLS performance skills during training and at 3 months.ResultsSome students (46%) knew how deep to push on an adult chest when doing CO-CPR before training. The course was associated with an increase in knowledge score (pretraining 55%, post-training 93%; adjusted mean difference (MD) 38%, 95% CI 33% to 43%; p<0.001). Most students (68%) thought that CPR education in senior secondary school was essential before training. The students had a very positive attitude towards CPR; no change in the mean (SD) attitude score out of 30 over time (pretraining 27.2 (2.5), post-training 27.6 (2.7); adjusted MD 0.5, 95% CI −0.1 to 1.0; p=0.132). Most students were competent in performing BLS immediately after training (77%) and at 3 months (83%) (adjusted MD 6%, 95% CI −4% to 15%; p=0.268).ConclusionsThe results demonstrate the feasibility of scaling up the number of secondary schools trained in a brief CO-CPRAED course within the local school curriculum.

Introduction: Bystander cardiopulmonary resuscitation (CPR) reduces mortality from out-of-hospital cardiac arrest. The willingness to perform CPR (W-CPR) is also critical. Uncertain effects of the coronavirus disease 2019 (COVID-19) pandemic on W-CPR were reported. Our objectives aim to examine W-CPR during the COVID-19 pandemic, including the influence of the bystander-victim relationship, bystander characteristics, and CPR background on the W-CPR of laypeople and healthcare providers (HCPs). Methods: A cross-sectional online survey was conducted between August 2020 and November 2020 among Thai laypeople and HCPs. A structured questionnaire was given to volunteers as an online survey. We recorded W-conventional CPR (W-C-CPR), W-compression-only CPR (W-CO-CPR), chest compression, automated external defibrillator (AED), mouth-to-mouth, face shield, and pocket mask ventilation on family members (FMs), acquaintances, and strangers during the study (pandemic) and in nonpandemic situation and analyzed. Results: We included 419 laypeople and 716 HCPs. During the pandemic, laypeople expressed less willingness in all interventions (P < 0.05) except W-CO-CPR in FMs and AED in FMs and acquaintances. HCPs were less willing to any interventions (P < 0.05). Laypeople showed comparable W-C-CPR and W-CO-CPR between FMs and acquaintances but less among strangers (P < 0.05). HCPs’ W-CPR differed significantly depending on their relationship (P < 0.05), except W-CO-CPR between FMs and acquaintances. CPR self-efficacy, single marital status, CPR experience, and HCPs reported higher W-CO-CPR in FMs. Conclusion: Participants were less W-CPR during the COVID-19 pandemic on all recipients (laypeople: 2.8%–21.0%, HCPs: 7.6%–31.2%), except for laypeople with FMs. The recipient’s relationship was more critical in W-C-CPR than in W-CO-CPR, especially in HCPs.

Background: Out-of-hospital cardiac arrest (OHCA) is a significant health problem in many Asian countries. Bystander cardiopulmonary resuscitation (CPR) can reduce mortality from OHCA. The willingness of bystanders to initiate aid is also critical. During the COVID-19 pandemic, CPR eases to spread of COVID-19 with negatively affect the decision. The objectives were to study the influence of the COVID-19 pandemic and bystander-victim relationship on the willingness of Thai healthcare providers (HCPs) and laypersons to perform CPR and associated factors.Methods: The cross-sectional online survey was conducted between August to November 2020 in a Thai population. A structured questionnaire was developed and given to volunteers as an online survey. We recorded participant characteristics and willingness to perform compression-only CPR (W-COC) and conventional CPR (W-CC) on family members, acquaintances, and strangers during the pandemic and in a non-pandemic situation and analyzed associated factors.Results: We included 419 laypersons and 716 HCPs. Lay-participants expressed less W-COC on acquaintances and strangers (risk ratio [RR] 0.74-0.85), but not on family members. By contrast, the HCPs were less W-COC across the board (RR 0.84-0.92). Both groups were less W-CC on all recipients (RR 0.43-0.54). There was no difference in participants’ W-COC and W-CC on family members and acquaintances, except that HCPs expressed greater W-CC on family members. Participants in all groups were less W-COC and W-CC on strangers. W-COC was correlated with CPR knowledge (Odds ratio [OR] 2.32), self-efficacy (OR 1.96), single marital status (OR 1.91), and being an HCP (OR 2.00). W-CC on family members was related to CPR knowledge (OR 2.16).Conclusion: Participants were less willing during the COVID-19 pandemic on all groups except family members of lay participants. The victim’s relationship to the participant was more important in conventional CPR than compression-only CPR, especially in HCPs. CPR knowledge, self-efficacy, single marital status, and being an HCP related to higher willingness.Trial registration: TCTR20210330003 (Thai Clinical Trials Registry)
