Long-term health conditions can limit achievement of personal goals. We aimed to map and synthesize definitions of goal adjustment, theoretical underpinnings, associations with recovery and supportive interventions for adults with long-term conditions. We searched multiple databases (January 2007-June 2018) and identified peer-reviewed research relating to goal adjustment. Data were charted, mapped and synthesized using content analysis and descriptive summaries. Two stakeholder consultations informed the review. Ninety-one articles were included. A range of long-term conditions were represented including cancer (22%), stroke (12%) and mixed neurological conditions (8%). Goal adjustment was one available option when faced with unattainable goals; other options were goal disengagement and goal re-engagement. Most studies were quantitative (58%), reporting mainly positive associations between goal adjustment, disengagement, reengagement and recovery. The Dual Process Model, Goal Adjustment Model and Self-Regulation Theory were most cited underpinning models/theory. Five interventions were identified; only one (self-system therapy) was evaluated in a randomized controlled trial. Our review provides original and significant insights into goal adjustment definitions, theoretical underpinnings and association with recovery. Effective interventions to support goal adjustment, disengagement and reengagement are lacking. This research-practice gap warrants attention to ensure people with long-term conditions are optimally supported when facing unattainable goals.

Introduction The purpose of this study was to test the feasibility of a telehealth intervention combining computerised cognitive training and occupation-based treatment among adult cancer survivors experiencing cancer-related cognitive impairment. Method This was a single-arm study including six cancer survivors with cancer-related cognitive impairment. Participants completed the computerised retraining and functional treatment intervention consisting of 10 hours of computerised cognitive training (BrainHQ) and approximately 10 1:1 remote sessions with an occupational therapist using an occupation-based approach. Feasibility was measured in terms of recruitment, adherence, acceptability and potential effectiveness. Assessments were administered at baseline, post-intervention and at 3-month follow-up and included measures of occupational performance, objective cognitive functions, quality of life and mood questionnaires. Results From the nine participants enrolled in the study, six completed 8–13 weeks of computerised retraining and functional treatment intervention and reported high satisfaction levels. Positive and significant improvements were found in occupational performance, useful field of view and social quality of life (all P < 0.05). The positive gains were maintained at the 3-month follow-up. Conclusion A combined model of computerised cognitive training and occupation-based treatment delivered remotely to adults with cancer-related cognitive impairment was found feasible and may improve occupational performance, cognitive functions and quality of life.

Objectives: The current study examined how a technology system, "It's Never 2 Late" (iN2L), may help augment traditional rehabilitation strategies for older adults with dementia by improving engagement in therapy sessions and achieving better functional outcomes. Method: The study used a two group quasi-experimental design. Older adults with dementia (N ¼ 96) were recruited from two rehabilitation departments housed within residential care communities. Participants received daily occupational and physical therapy sessions using treatment as usual (TAU) at one site (n ¼ 49) or treatment with iN2L (n ¼ 47) at the other site. A goal attainment approach was used to assess functional outcomes. It was hypothesized that patients whose therapists used iN2L in treatment will show greater attainment of therapy goals and greater engagement during OT and PT sessions than patients receiving TAU. It was also hypothesized that levels and improvement in engagement will mediate the association of treatment type (iN2L or TAU) with greater goal attainment. Results: Participants in the iN2L treatment had significantly higher goal attainment than TAU, significantly higher levels of engagement at baseline, and significantly steeper increases in engagement over the course of therapy. The effects of treatment on goal attainment was significantly mediated by increases in engagement. Conclusion: Findings suggest that iN2L technology has the potential to increase treatment engagement and enhance rehabilitation outcomes among older adults with dementia. ARTICLE HISTORY
