GnRH Agonist versus hCG Trigger in Ovulation Induction with Intrauterine Insemination: A Randomized Controlled Trial

Abstract: This study is aimed at comparing clinical pregnancy rates (CPRs) in patients who are administered either gonadotropin-releasing hormone agonist (GnRHa) or human chorionic gonadotropin (hCG) for ovulation trigger in intrauterine insemination (IUI) cycles. A prospective randomized comparative study was conducted at Hue University Hospital in Vietnam. A total of 197 infertile women were randomly assigned to receive either GnRHa trigger (n=98 cycles) or hCG trigger (n=99 cycles) for ovulation trigger. Patients ret… Show more

“…There are some plausible biological mechanisms to support our findings concerning SBP. Medications to induce ovulation, such as gonadotropins and GnRH agonists, are used to achieve supraphysiological levels of estradiol [ 23 ]. Estrogens affect vascular health and blood pressure [ 24 ].…”

“…Other than HCG, GnRH-a has been used as an alternative to induce ovulation in the last two decades. GnRH agonists induce the endogenous surge of LH and FSH by their flare-up effect, which is helpful in triggering ovulation and decreasing the risk of OHSS [ 1 3 ]. Ovulation triggers with hCG or GnRH are widely used in the IUI cycle.…”

“…These studies reveal that the final maturation of oocyte with dual trigger improves the number of mature oocytes (MII) and oocyte quality, which is reflected by the higher number of good quality embryos, thus improving the clinical pregnancy rate. HCG has traditionally been used as the gold standard for triggering oocyte maturation in controlled ovarian stimulation cycles [ 1 3 , 24 ]. GnRH-a is one of the triggers of final oocyte maturation that is used as an alternative to hCG to prevent OHSS in PCOS patients [ 16 19 ].…”

“…Human Chorionic Gonadotropin (hCG) can be used as a trigger; in cases in which an antagonist is used in pituitary block, a GnRH agonist should be chosen ( Alyasin et al ., 2016 ; Le et al ., 2019 ). hCG is a gonadotrophic glycoprotein hormone, produced mainly by placental trophoblasts, as the LH produced by the pituitary gland and used in the final maturation of oocytes.…”
