Glomerular filtration rate (GFR) in patients with stage I testicular seminoma treated with adjuvant carboplatin: A comparison of six formulae compared to a radioisotope gold standard

Abstract: 15504 Background: Carboplatin (Carbo) is an effective adjuvant treatment for patients (pts) with stage I testicular seminoma. Accurate estimation of GFR is necessary to establish carbo dose. Calculation of GFR following injection of a radiolabelled isotope (e.g. Tc99mDTPA) is considered the gold standard, but may not be readily available in all centres. Calculation of creatinine clearence via 24-h urine collection is often unreliable. The aim of this study was to assess different formulae for GFR calculation … Show more

“…Although the confidence intervals are wide, reflecting small numbers and great inter‐individual variation in GFRs as is in seen in a clinical population, the magnitude and consistency of the differences are clinically significant. The CG is a formula frequently used for the prediction of kidney function and has been validated for drug dosing 11,17,35 . However, with time practitioners have developed a cautious approach when using this formula in various hospitalized patients because of the unpredictable relationship of muscle catabolism to renal creatinine clearance.…”

“…Patients who experience significant nausea or vomiting with one of these regimens should receive an agent from a different pharmacologic category. 1316 There is no evidence that substituting granisetron for ondansetron in subsequent treatment cycles, or increasing the dose, even to very high doses, is effective. This approach is not recommended.…”
