Global, regional, and subregional trends in unintended pregnancy and its outcomes from 1990 to 2014: estimates from a Bayesian hierarchical model

Abstract: Summary Background Estimates of pregnancy incidence by intention status and outcome indicate how effectively women and couples are able to fulfil their childbearing aspirations, and can be used to monitor the impact of family-planning programmes. We estimate global, regional, and subregional pregnancy rates by intention status and outcome for 1990–2014. Methods We developed a Bayesian hierarchical time series model whereby the unintended pregnancy rate is a function of the distribution of women across subgr… Show more

“…Worldwide, unplanned pregnancies constitute a major women’s health problem. In a global perspective, 23% of all births and 44% of all pregnancies are estimated to be unintended . Unplanned pregnancies expose women to unnecessary health risks that come with pregnancy, terminations of pregnancy and birth.…”

“…In a global perspective, 23% of all births and 44% of all pregnancies are estimated to be unintended. 1 Unplanned pregnancies expose women to unnecessary health risks that come with pregnancy, terminations of pregnancy and birth. They may cause suffering, disabilities and death due to unsafe termination methods and pregnancy complications; they have costs for society, and are associated with maternal postpartum depression.…”

“…Being the leading form of preventable mental retardation and having lifelong effects that can severely reduce the quality of life of those affected (61), it is critical that we fully understand the impact that all levels of ethanol exposure can have. The effect of moderate levels of alcohol exposure are becoming even more crucial with the number of women having unplanned births and pregnancies being as high as 23% and 40% respectively (62,63), worldwide. Therefore, the chance of drinking before becoming aware of being pregnant is a huge risk factor.…”

“…6,7 While there is no association found between the legal status of abortion and total abortion rates, 7 in restrictive settings 75% of the abortions are unsafe, compared with 12.5% in countries without restrictions. 11 In the past two decades, international human rights bodies and United Nations (UN) expert committees have characterized unsafe abortion or lack of access to safe abortion as a human rights issue concerning violations to girls and women's rights to health; privacy; non-discrimination; and freedom of cruel, inhuman, and degrading treatment. [8][9][10] This is despite a decline in unintended pregnancies.…”

“…Studies show that a reduction of unintended pregnancies in the developing regions does not coincide with a reduction in abortion rates, indicating that besides a need for contraceptive services, the need for safe abortion care remains. 12,13 Abortion has become medically more feasible and safer with the increased availability and use of misoprostol and mifepristone.…”
