Global perspectives of emerging occupational and environmental lung diseases

Abstract: Clinicians must remain vigilant for potential occupational and environmental exposures, especially when evaluating patients with unusual and unique presentation, so that occupational and environmental risk factors may be identified, and monitoring and preventive measures can be implemented early.

“…Iako je pušenje cigareta najbolje proučen faktor rizika , i aerozagađenje, zadnjih godina postoje i studije koje dokazuju da i nepušači mogu da razviju opstrukciju sa ograničenjem protoka vazduha.Genetski faktori, kao što je deficit alfa -1 -antitripsina, zatim gen koji kodira matriksnu metiloproteinazu (MMP12), dovode se u vezu sa opadanjem plućne funkcije [3,5,6 ]. Inflamacija u plućima kod obolelih od HOBP-a je najverovatnije reakcija respiratornog trakta na hronične iritanse poput duvanskog dima.Oksidativni stres i višak proteinaza takođe modifikuju inflamaciju u plućima.Specifični obrazac inflamacije se karakteriše i povećanjem broja CD8+ (citotoksičnih )limfocita.Veliki broj medijatora zapaljenja privlači inflamatorne ćelije i iz cirkulacije, čime se pojačava zapaljenski proces i indukuju strukturne promene.Broj inflamatornih ćelija se povećava i u perifernoj krvi.…”
