Global and Local Epidemiology of Community-Acquired Pneumonia: The Experience of the CAPNETZ Network

Abstract: Community-acquired pneumonia (CAP) is a major cause of morbidity and mortality worldwide. Young children and the elderly are disproportionately affected by CAP. Lower respiratory tract infections (LRTIs), including CAP, were ranked third in a list of the 30 leading causes of death worldwide in 1990. Mortality rates are low (< 2%) in CAP patients treated as outpatients, but are higher (5 to 20%) among patients hospitalized for CAP, and are highest (up to 50%) in patients admitted to the intensive care unit. “…Ewig et al reported the incidence of communityacquired pneumonia requiring hospitalization by age group based [58]. It was estimated that approximately 40% of communityacquired pneumonia cases were attributable to pneumococcal infection, based on the German Competence Network for Community-Acquired Pneumonia study [59]. Case-fatality rates also came from the Competence Network for CommunityAcquired Pneumonia study [59].…”

“…The ratio of C-reactive protein to procalcitonin significantly distinguished between the groups. High procalcitonin levels were associated with an adverse clinical course.Community-acquired pneumonia (CAP) may be caused by either Streptococcus pneumoniae or Legionella pneumophila (5,19). The initiation of adequate empirical antimicrobial therapy can be challenging, since S. pneumoniae and L. pneumophila have partly contrasting antimicrobial susceptibility patterns (11).…”

“…A recent review by Welte and Kohnlein (in 2009) showed a mortality of patients hospitalized with CAP of 5 to 20% (20). Treatment failure due to antimicrobial resistance may account for at least a part of that.…”
