Objectives As facial aging occurs, aesthetic changes of the periorbital region include eyebrow ptosis, sagging in lateral temporo-orbital region with superior eyelid hooding, ptosis of orbicularis oculi muscle associated to drooping of malar area with multiple "crow feets", a deeper lid-cheek junction and malar festoons. All theese periorbital structures should be considered as a single anatomical entity Methods Numerous techniques have been described to correct brow aesthetics in facial rejuvenation. We report the senior author’s (M.P.) current approach utilizing a temporal subcutaneous brow lift (TSBL) associated to an orbicularis oculi muscle (OOM) elastic flap. Furthermore, a systematic review of the literature was performed comparing the different surgical approaches striving to clarify its terminology. A total of 298 patients underwent this procedure in 4 years period. Of theese, 230 patients, with at the least 12 months of follow-up, were submitted to subjective and objective methods to evaluate the self perception of scar and their overall aesthetic satisfaction. A Committee of experts, blinded to each other’s assessment, evaluated the same outcomes. A mesure of amount of eyebrow's tail lifting was recorded for each patients. Results The most satisfying aspect of these techniques is dramatic periorbital rejuvenation and preservation of the patient’s original youthful identity. In fact the postoperative results appear very natural and do not suffer from an “operated” look. This aspect is noticeable from both the surgeon’s and the patient’s perceptions. There were no major complications in any of the cases. Conclusions The proposed TSBL with OOM elastic flap technique, in the hands of senior author (M.P.) has emerged as a reliable, effective, and highly reproducible method to treat sagging and aging of the lateral region of the orbit, even without associated blepharoplasties. The submental-cervical angle is an important anatomical landmark in neck beauty. Considered attractive and a sign of youthful when between 105° and 120°, greater values are considered a “heavy” neck or double chin and are related to the aging process and/or weight gain. The submandibular gland can also contribute to the alteration of the submental-cervical angle, increasing the area’s bulging. Neck-lifting techniques have the potential to produce important changes in the lateral view of the face, making it look more youthful, and that is more noticeable in the frontal aspect. This review focuses on the treatment of the neck, including all modifications that occur during the aging process, and postoperative procedures used to decrease the risk of complications.

The design of the upper third of the face is one of the most important aspects that the facial plastic surgeon must be concerned about to achieve good aesthetic results during rejuvenation procedures.Different approaches are described, from coronal foreheadlift to endoscopic eyebrow-lift. 1,2 Before choosing the best technique, one must evaluate the position and quality of the hairline, eyebrow position, skin mobility, bone structure, symmetry, and shape of the periorbital area.The exact position of the eyebrow and its maintenance postoperatively are the main concerns. These should be in mind especially in women, whose eyebrows must be arched with a peak corresponding to the lateral aspect of the eye, whereas men's eyebrows can present a more horizontal design. 3 We describe an open surgical approach to elevate the eyebrows through small incisions, subcutaneous frontal detachment, and plication of the frontal muscle. The technique can be viewed in the Video.
