Trauma, both blunt and penetrating, is extremely common worldwide, as trauma to major vessels. The management of these patients requires specialized surgical skills and techniques of the trauma surgeon. Furthermore few other surgical emergencies require immediate diagnosis and treatment like a ruptured abdominal aortic aneurysm (rAAA). Mortality of patients with a rAAA reaches 85 %, with more than half dying before reaching the hospital. These are acute events demanding immediate intervention to save life and limb and precluding any attempt at transfer or referral.It is the purpose of this position paper to discuss neck, chest, extremities and abdominal trauma, bringing to light recent evidence based data as well as expert opinions; besides, in this paper we present a review of the recent literature on rAAA and we discuss the rationale for transfer to referral center, the role of preoperative imaging and the pros and cons of Endoluminal repair of rAAA (REVAR) versus Open Repair (OR).

Kite flying is a common activity in many countries, particularly India. Fierce competition has led to the use of dangerous kite threads like manja (which is a cotton or nylon thread coated with powdered glass) to gain an edge over rivals. The sharp manja can not only cause linear abrasions or deep incised wounds among flyers but it can inflict equally serious injuries to onlookers or unwary pedestrians and two-wheeler riders on the street. Hand, throat, angle of mouth, nose, and feet are some common sites of injury. We report a case of a 62-year-old man who sustained a manja cut injury through the tendoachilles while walking on the roadside when his foot got entangled in the barely visible thin thread as a bicycle rushed past him. The wound was debrided and tendoachilles repaired using the Krackow technique. The patient had reasonable strength in plantar-flexion and good active range of motion at the ankle when last seen at the one-year follow-up. The aim of this article is to highlight the hazards of manja and the need to revisit and acknowledge the clinical, social, and administrative implications of this unusual but noteworthy mechanism of limb and lifethreatening injury.

Achilles tendon is the strongest and the thickest tendon of the human body. Due to its superficial location and stress, it has to face due to loading, the tendon is susceptible to injuries and ruptures. This is aggravated due to the precarious blood supply in the region and also age-related changes that occur in the tendon. Of the specific causes age-related and steroid induced tendinosis and sports injuries are the commonest mechanism of injury. Kite playing is a very common sport in the Indian subcontinent, which involves the use of a string (manja) often coated with powdered glass to fly the kite. Many grievous injuries are reported due to its use especially in two wheeler riders. However, to the best of our knowledge a kite thread injury causing Achilles tendon tear due to direct impact has so far not been reported.
