Gingival depigmentation: A split mouth comparative study between scalpel and cryosurgery

Abstract: Gingival hyperpigmentation is a major esthetic concern for many people. Although it is not a medical problem, many people complain of dark gums as unesthetic. Gingival depigmentation is a periodontal plastic surgical procedure, whereby the hyperpigmentation is removed or reduced by various techniques. For depigmentation of gingival, different treatment modalities have been reported, such as scalpel, cryosurgery, electrosurgery, lasers, etc., this article compares the management of three cases with scalpel and … Show more

“…This was consistent with several clinical trials reporting the favorable clinical outcomes with the surgical stripping technique. 13 Corresponding with Patil et al, 11 The distribution of repigmentation in the presented case was patchy but did not affect the overall satisfaction level of the patient. The occurrence of re-pigmentation was noticed at the third month following the surgical intervention.…”

“…10 Other than LASER, using cryosurgery would have been inconvenient due to the lack of control over the duration and depth of freezing as well as the reported complication of postoperative swelling. 11 Electrosurgery, as well, have the disadvantage of unfavorable excessive tissue destruction, as well as more inflammatory response of the tissues compared to scalpel wounds. 12 In the presented case, the depigmentation step succeeded to reduce the pigmentation score from 3 to 1 according to the Dummett-Gupta Oral Pigmentation Index.…”

“…Gingival abrasion technique using a large round diamond bur in a high-speed handpiece with abundant irrigation has also been practiced [ 9 ]. The main problem with this technique is the difficulty in controlling the depth of the depigmentation and obtaining adequate access [ 19 ].…”

“…There seems to be a lesser amount of post-op pain for cryosurgery and laser surgical procedures compared to other gingival depigmentation techniques. [9][10][11][12][13][14][15] The lesser amount of postop pain for the laser is due to the coagulation of protein on the wound surface that serves as a biological wound dressing. In addition, the laser has the ability to seal the sensory nerve endings.…”

“…In the present study, the laser was also a better option compared to cryosurgery. Patil et al 15 used 940 nm diode laser to treat an 18-year-old woman who suffered from gingival physiologic hyperpigmentation. After 1 week, the gingiva had no pigmentation, but after 6 months, pale patches appeared, and after 1 year the recurrent pigmentation was larger.…”
