Gianturco expandable wire stents in the treatment of superior vena cava syndrome recurring after maximum-tolerance radiation

Abstract: Two patients with superior vena cava syndrome (SVCS) recurring after maximum-tolerance radiation were treated by placing a Gianturco expandable wire stent (GEWS) into the obstructed superior vena cava. The SVCS symptoms rapidly disappeared and good short-term (6 months) palliation was achieved. GEWS placement is a promising therapeutic alternative for palliation of SVCS symptoms when other therapeutic modes cannot be used or are not effective.

“…In addition to use in the larger pelvic and femoral arteries, stents are now also implanted in the arteries of the lower leg. As a result of the success of stents in arteries, they have also been used in venous stenoses initially in animal experiments and later in humans [5,6].…”

“…The introduction of expandable metal vascular stents further broadened the spectrum of closed minimally invasive techniques in the chest, allowing for minimally invasive treatment of superior vena cava (SVC) obstruction with an immediate clinical result [13]. The most prominent interventional radiological techniques in the chest mentioned earlier will be discussed in this paper.…”

“…(27)(28)(29)(30)(31)(32) Regardless of the type of stent used, immediate relief of symptoms such as headache has been reported by various authors. Cyanosis and facial edema improve in one or two days, (31)(32)(33)(34) and edema of the upper limbs is generally resolved two or three days after the insertion of the stent (23,30,34) but can persist for up to one week.…”

“…The authors observed immediate relief of the obstructive symptoms, as well as a favorable palliative result in the short-term (six months), in both patients. (31) In fact, prior to that study, the Gianturco stent had been used in the treatment of SVCS by other authors (in 1986). However, in that previous study, the patient died (as a result of chemotherapy toxicity) three weeks after stent placement.…”
