Giant saccular distal azygos artery aneurysm: Report of a case and review of literature

Abstract: An azygos anterior cerebral artery (ACA) is a rare variant of normal embryogenesis in which confluence of two A1 segments results in a single A2 segment with the absence of anterior communicating artery. The occurrence of an aneurysm at the bifurcation of azygos ACA is rare with few cases reported in the literature. We report a case of a 40-year-old lady who presented with subarachnoid hemorrhage following rupture of a giant, saccular distal azygos ACA aneurysm. Bifrontal craniotomy and clipping of an aneurysm… Show more

“…[ 13 ] Second, we did not measure the concentrations of inflammatory cytokines either in the blood or cerebrospinal fluid. Finally, a bilateral frontal craniotomy together with resection of the falx cerebri may have provided a wider surgical field and better exposure of the aneurysm,[ 14 ] thus preventing postoperative perianeurysmal edema. We were concerned about his poor cardiac function; therefore, we performed a relatively small craniotomy and surgical exposure with the hope to shorten operation time.…”

“…The azygos anterior cerebral artery (ACA) is a very uncommon anomaly of the circle of Willis with an incidence of 1–4%. [ 1 3 7 8 18 ] In this variant of normal embryogenesis, the confluence of both A1 segments results in the formation of a single distal ACA in the interhemispheric fissure. [ 1 3 8 ] This unique vascular configuration is frequently associated with saccular aneurysms and has significant impact on the arterial hemodynamics of the frontal lobe.…”

“…The literature also provides a description of the coexistence of an azygos artery with corpus callosum hypoplasia, porencephalia, or vascular malformation [1][2][3][4]. In my own extensive (material subsequently treated by clipping and reviewed patients after subarachnoid haemorrhage within the last 10 years, no case of an azygos anterior artery A2 segment and coexisting giant aneurysm has been found.…”

“…This was the only case of a giant aneurysm located in the azygos pericallosal artery. In the literature of the subject only a handful of cases have been reported on the location and surgical treatment of giant aneurysm in azygos pericallosal artery [4,[6][7][8][9][10].…”
