Giant right atrial diverticulum: an unusual cause of Wolff-Parkinson-White syndrome

Abstract: A case of a giant right atrial diverticulum associated with neonatal supraventricular tachycardia is reported. The electrocardiogram in sinus rhythm showed pre-excitation that may have been caused by the right atrial diverticulum adhering to the right ventricle.Congenital diverticulum or aneurysm of the right atrium is a very rare anomaly. Morrow et al reported the case of a 23 year old woman with a large congenital aneurysm of the right atrium that caused repeated attacks of supraventricular tachycardia and w… Show more

“…Smooth and communicating diverticula are suggested to create a potential "loop" to trigger a transmission disorder (8,9,11,12). As coronary CTA is problematic in patients with disordered cardiac rhythms such as atrial fibrillation (13), these patients were excluded from our study, and thus the relationship between atrial fibrillation and the frequency of atrial diverticula could not be evaluated.…”

“…Several thrombi were suspected to lie within the diverticulum on echocardiography of this case although D‐dimer level was in the reference range. The diverticulum can be so large that it compresses the right atrium and ventricle 15. This case showed serious clinical signs associated with congestive heart failure.…”

“…4 Right atrial diverticula have all been associated with supraventricular tachycardia. 4,6 It is suggested that abnormal tissue in the wall contains irritable ectopic foci or may provide extensive surfaces over which abnormal circular impulses are transmitted. 4 In one patient, the tachycardia was thought to be caused by adherence of the giant diverticulum to the under- lying right ventricle.…”

“…4 In one patient, the tachycardia was thought to be caused by adherence of the giant diverticulum to the under- lying right ventricle. 6 The diverticula have been successfully excised from the heart in some patients, resulting in restoration of normal cardiac rhythm. 4 Although an electrocardiogram was not performed during clinical examination of this foal, the heart rate was reported to be increased.…”
