Giant pedunculated oesophageal liposarcomas: A review of literature and resection techniques

Abstract: HighlightsGiant oesophageal liposarcomas, a rare but important cause of oesophageal tumours.A review of the current literature focusing on resection techniques.Shift in the treatment paradigm in recent years to endoscopic resection techniques.Decision on type of resection technique depends on tumour characteristics and location.

“…Sixty-three patients with esophageal liposarcoma were found, 62 from a review [ 74 ] and one more recent case report [ 73 ]. Most liposarcomas were polypoid; in 10 patients, an endoscopic approach was used, whereas 9 patients were treated through a left cervical incision.…”

“…Esophagectomy may be indicated for either tumors without a stalk or tumors with a wide stalk. Endoscopic resection is considered to be the least invasive procedure, but there are some limitations to its use as following after: first, the tumor is pedunculated; second, there is a single stalk; and third, there are no large vessels in the stalk [6]. In the case of endoscopic resection, when the tumor is too large for picking out orally, it must be removed through a cervical incision [4].…”

“… 6 The reported treatment of oesophageal liposarcoma mainly includes transthoracic oesophagectomy, transoral resection and endoscopic submucosal dissection, along with total esophagectomy. 7 It is not possible to resect large oesophageal tumours with endoscopic submucosal dissection. 5 However, compared with open surgery, thoracic laparoscopy can safely and effectively resect large oesophageal masses and reduce the surgical duration, intraoperative blood loss, lymph node dissection and postoperative complications.…”
