Giant mediastinal teratoma presenting with paroxysmal atrial fibrillation

Abstract: Mediastinum is a common site where benign tumors, like teratomas, can develop. Usually, these lesions do not cause any symptoms and the diagnosis is reached accidentally. As they enlarge they may cause symptoms by compressing the nearby structures of the thorax, mostly the trachea and the bronchi. Extrinsic compression of the heart or the great vessels appears to be a very rare occurrence. Atrial fibrillation as the first clinical presentation of left atrial compression by a giant mediastinal teratoma is extre… Show more

“…Semptomlar genellikle çevre dokulara bası ile ortaya çıkar. Literatürde hemoptizi, plevral efüzyon, perikardiyal efüzyon, paroksismal atriyal fibrilasyon, saç ekspektorasyonu gibi bulgularla başvuran hastalar mevcuttur (5,7,8) . Olgumuz, olasılıkla lezyonun mediastinal kistik komponentlerinden birinin perfore olması ile gelişen bir ampiyem tablosunda başvuruda bulundu.…”

“…Çoğunlukla çevre yapı-lara bası yaptıktan sonra belirti verdiklerinden dolayı, erken tanı ve tedavi enderdir (4) . Kesin tedavi komplet cerrahi rezeksiyondur (5) . Bu olgu sunumunda, radyolojik olarak invaziv görünümde olan ve inoperable olarak değerlendirilen bir matür teratom olgusunun, cerrahi olarak tedavi edilmesi bildirilmiştir.…”

“…The question is “How can a brain tumour affect the heart?” The mechanism is unclear, but one theory involves compression of the heart by intrathoracic metastases, leading to arrythmias. Indeed, AF is known to be induced by external pressure from intrathoracic tumours or masses,1315 and GBM has been shown to metastasise to the thoracic regions such as lymph nodes,16 lungs17 18 and pleura 17 18. So on face value, extracranial GBM metastases to the thorax causing arrhythmias is feasible.…”
