RESUMEN:El tratamiento del hemangioma hepático gigante (HHG), sigue siendo motivo de controversia. El objetivo de este estudio fue reportar los resultados de pacientes con HHG resecados quirúrgicamente en términos de morbilidad postoperatoria (MPO). Serie de casos con seguimiento. Se incluyeron pacientes con HHG, sometidos a cirugía de forma consecutiva, en Clínica RedSalud Mayor, entre 2011 y 2020. La variable resultado fue MPO. Otras variables de interés fueron: tiempo quirúrgico, estancia hospitalaria y mortalidad. Las pacientes fueron seguidas de forma clínica. Se utilizó estadística descriptiva, con medidas de tendencia central y dispersión. Se intervinieron 5 pacientes, con una mediana de edad de 38 años. La medianas del tiempo quirúrgico y estancia hospitalaria; fueron 75 min y 4 días respectivamente. La MPO fue 20 % (1 caso de seroma). Con una mediana de seguimiento de 41 meses, los pacientes se encuentran asintomáticos y no se ha verificado morbilidad alejada. La resección quirúrgica de un HHG se puede realizar con escasa morbilidad, tanto en términos numéricos como de gravedad de la complicación observada.

Đặt vấn đề: U máu gan là khối u lành tính phổ biến nhất ở gan, nếu khối u có kích thước lớn thì phải có chỉ định điều trị phù hợp. Mục tiêu: Đánh giá chỉ định điều trị và kết quả mô bệnh học của bệnh nhân u máu gan. Phương pháp: Nghiên cứu cắt ngang trên 49 bệnh nhân được chẩn đoán u máu gan dựa trên hướng dẫn của Hiệp hội Nghiên cứu về Gan Châu Âu. Trên hình ảnh CT có tiêm thuốc cản quang, khối u gan có hình ảnh ngấm thuốc ngoại vi pha động mạch, tăng cường hướng tâm trong pha chậm; chẩn đoán bằng mô bệnh học khi u không ngấm thuốc điển hình trên CT. Kết quả: RFA(Radiofrequency Ablation-đốt sóng cao tần) l là phương pháp điều trị thực hiện nhiều nhất với tỉ lệ 34,7%. Cắt khối u và TAE (Transcatheter arterial chemoembolization) cùng với tỉ lệ 14,3%. Cắt u được thực hiện ở lứa tuổi trẻ nhất 42,4 ± 10,9, nhóm RFA là 48,2 ± 11,4; nhóm TAE là 64,8 ± 23,3; p = 0,01. Kích thước u trung bình của nhóm cắt u là lớn nhất 7,1 ± 2,7 cm, nhóm RFA có kích thước nhỏ nhất 5,8 ± 2,6 cm, kích thước u nhóm TAE là 6,6 ± 1,8 cm, p = 0,01. Kích thước u trung bình của nhóm sinh thiết bằng kim là 3,3 ± 0,9 cm, nhỏ hơn so với nhóm sinh thiết bằng cắt lạnh 7,1 ± 2,7 cm, p = 0,009. Trong 13 bệnh nhân được sinh thiết, tỉ lệ lành tính là 100%. U máu thể hang chiếm tỉ lệ 69,2%, u máu thể mao mạch chiếm tỉ lệ 30,8%. Kết luận: Có thể thực hiện cắt bỏ, đốt nhiệt hoặc tắc mạch cho bệnh nhân u máu gan tùy từng trường hợp. U máu gan có kích thước lớn thì bản chất vẫn là lành tính.
