Employing new and original survey data collected in three waves (April/May and November 2020 as well as May 2021) in Germany, this paper studies the dynamics of individual-level support for additional health care spending. A first major finding is that, so far, health care spending preferences have not radically changed during the Covid-19 pandemic, at least at the aggregate level. A more detailed analysis reveals, secondly, that individual-level support for additional spending on health care is strongly conditioned by performance perceptions and, to a lesser extent, general political trust. Citizens who regard the system as badly (well) prepared to cope with the crisis are more likely to support (oppose) additional spending. Higher levels of political trust are also positively associated with spending support, but to a lesser degree. The paper concludes by discussing the implications of these findings for policy-making and welfare state politics in the post-pandemic era.

ZusammenfassungEs mangelt in Deutschland an institutioneller und regulativer Struktur sowie Koordination für Public Health. Die aktuellen Reformansätze beim öffentlichen Gesundheitsdienst (ÖGD-Pakt), zur Gründung eines Bundesinstituts für öffentliche Gesundheit und zur Novellierung des Präventionsgesetzes bieten Chancen, im Rahmen dieser Reformen die Fundamente für eine moderne Public Health-Struktur zu legen. Dafür werden in der vorliegenden Ausarbeitung, angelehnt an das Handlungsfeld Gesundheitsförderung und Primärprävention, die fünf Aufgabenbereiche 1. sozialepidemiologische Daten, 2. Gesundheitskommunikation, 3. Interventionsdurchführung, 4. Methodenentwicklung, Evaluation und Qualitätsentwicklung und 5. Diskursivierung skizziert, die sowohl für die praktische Arbeit aller identifizierten Akteure als auch für ihre Koordinierung von hoher Relevanz sind. Zusammengenommen eröffnet dies ein Fenster der Möglichkeiten für eine kohärente, flächendeckende, handlungs- und lernfähige Infrastruktur für Public Health in Deutschland.

Prevention and health promotion are important areas of welfare state activity and can be considered parts of the critical infrastructure. They have been considerably expanded in Western welfare states in recent years. In the health insurance states of Germany, Switzerland and Austria, new forms of organisation have emerged. The article describes the evolution and status quo of the organisation of prevention and health promotion in the three countries and explores the legitimisation patterns for the chosen institutional forms. To this end, health reforms, debates and statements of key stakeholders are analysed. A distinction is made between ‘normative’ legitimisation patterns and ‘functional’ ones that indicate a ‘social investment’ strategy. In Germany, the 2015 Prevention Act created an institutional structure in which the actors involved cooperate closely. It also gives the health insurance funds a prominent role. In Switzerland, the cantons are responsible for prevention and health promotion; at federal level the main bodies are the Federal Office of Public Health (BAG) and the Swiss Foundation for Health Promotion (Gesundheitsförderung Schweiz). In Austria, the Länder are largely responsible, but the federal level gained importance by establishing Gesundes Österreich GmbH and strengthening coordination. While the term ‘social investment’ is not encountered in the debates and documents analysed, many arguments commonly associated with it are increasingly used in the context of prevention and health promotion. In contrast, normative justifications seem to be losing importance.
