Abstract. Immigrants may be carriers of infectious diseases because of the prevalence of these diseases in their country of origin, exposure during migration, or conditions during resettlement, with this prevalence being particularly high in sub-Saharan Africans. We performed a retrospective review of 180 sub-Saharan immigrants screened for infectious diseases at an International Health Center from January 2009 to December 2012. At least one pathogenic infectious disease was diagnosed in 72.8% patients: 60.6% latent tuberculosis infection, 36.8% intestinal parasites (intestinal protozoa or helminths), 28.1% helminths, 14.8% hepatitis B surface antigen positive, 1.2% anti-hepatitis C virus positive, 1.2% human immunodeficiency virus-positive, and 1.2% malaria. Coinfections were present in 28.4%. There was significant association between eosinophilia (absolute count or percentage) or hyper-IgE and the presence of helminths (P < 0.001). Relative eosinophilia and hyper-IgE were better indicators of helminth infection than absolute eosinophilia, particularly for schistosomiasis and strongyloidiasis. We found a high prevalence of infectious diseases in sub-Saharan immigrants, which could lead to severe health problems (in the absence of prompt treatment), representing a high cost to the public health system and possible transmission in the host country. Accurate screening and tailored protocols for infectious diseases are recommended in sub-Saharan immigrants.

El advenimiento de las terapias biológicas para el manejo de múltiples enfermedades ha estado asociado con la reactivación o susceptibilidad a ciertas entidades infecciosas durante el uso de estas. El estudio de la fisiopatología del asma ha permitido llegar al reconocimiento de los diferentes mecanismos distintivos de esta enfermedad, facilitando así el desarrollo de terapias personalizadas que apunten al proceso sistémico subyacente de la enfermedad y resulten en un mayor beneficio para el paciente. Hasta el momento, estas terapias han demostrado un gran impacto en la mejoría de los síntomas y la calidad de vida de los pacientes; sin embargo, se empieza a evidenciar que, con su uso, algunos pacientes presentan con frecuencia enfermedades de origen infeccioso, como las infecciones respiratorias de origen viral, y susceptibilidad a parasitosis, como las más relevantes, de acuerdo con el mecanismo de acción de la terapia biológica empleada en el asma.En esta revisión narrativa de la literatura, se describen los hallazgos más recientes encontrados sobre el perfil de seguridad infecciosa en el uso de omalizumab y mepolizumab, como terapia biológica con mayor estudio para el manejo del asma severo refractario, con el fin de dar a conocer los riesgos infecciosos de estas moléculas y generar algunas recomendaciones para la prevención y el manejo de estas en nuestro contexto latinoamericano.Palabras clave: asma; terapia biológica; anticuerpos monoclonales; omalizumab; mepolizumab. AbstractThe advent of biological therapy for the treatment of multiple diseases, has been related to the reactivation or susceptibility to certain infectious entities during its use. The understanding of asthma physiopathology has led to the recognition of diverse and distinctive mechanisms of the disease, allowing the development of targeted biological therapies directed to the underlying systemic disease process and resulting in a greater benefit for patients. Until present, these therapies have demonstrated a pronounced impact on symptoms improvement and on patient life quality; however, it is now being plausible the appearance of infectious diseases, such as viral respiratory infections and susceptibility to parasitism, among the most important illnesses related to mechanism of action of biological therapies used in asthma.In this narrative review we described the most recent findings found about infectious safety profile for the use of biological therapy in asthma with Mepolizumab and Omalizumab, being those two the most used biological therapies used in treatment of severe refractory asthma; with the purpose of announce the infectious risks of this molecules and generate some recommendations for prevention and management of this risks in our Latinamerican context.

Determination of Geohelminthiasis and Its Association with Allergic Sensitization among Selected Children in Batangas, Philippines

Several studies have explored the cross-reactivity between geohelminthiasis and allergy which share the same immune response in humans by triggering the increase in serum IgE level. Yet, the role of soil-transmitted intestinal parasitism as a protective or contributing factor for allergy remains inconclusive. In this study, the association of geohelminthiasis with allergic sensitization was investigated in a cohort of 50 children aged 5 to 12 years old residing in the areas of Batangas, Philippines where parasitism is prevalent. Stool samples of the subjects were qualitatively examined for the presence of helminth infection through modified Kato-Katz technique. They were also assessed for allergic diseases using the modified International Study of Asthma and Allergies in Childhood Phase I questionnaire. Fecalysis results were correlated with their serum total IgE (tIgE) levels and screening test results for allergic sensitization to common aeroallergens and food allergens determined through ImmunoCAP ® system. Of the participants, 78% were under 10 years old while 22% were aged 10 years old and above. Mixed infection of Ascaris lumbricoides and Trichuris trichiura were found to be the most common (40%) followed by trichuriasis (34%) and ascariasis (26%). Atopic allergy to inhalants is prevalent in children under 10 years old (74.36%) whereas, some of them (5.13%) are fX2 positive which confirms the participants' allergy to seafood. It was revealed that the occurrence of geohelminthic infection and allergy may affect any child regardless of age and serum total IgE level does not considerably vary with age, geohelminthiasis and presence of allergy. Hence, geohelminthiasis does not play contributory nor inhibitory role in allergic sensitization as established by a 0.05 level of significance (p≤0.05).
