Ensuring widespread access to healthcare services is a crucial policy objective. The Italian National Recovery and Resilience Plan (NRRP) has notably prioritized territorial medicine, channeling post-pandemic investments towards the restructuring of primary care services. A significant change involved the establishment of Community Healthcare Centers (CHCs). This paper investigates how CHCs contribute to accessibility to healthcare in urban and rural areas. By leveraging a comprehensive dataset of general practitioners’ availability and estimating supply and demand future scenarios, we examine the impact of CHCs under two different capacity allocation strategies. Strategy 1—Capacity expansion—involves allocating additional service hours of general practitioners in CHCs to maximize accessibility. Strategy 2—Capacity redistribution—accounts for the persisting shortage of healthcare professionals faced by Italy in recent years, thereby reallocating a portion of general practitioners’ current services from their existing workplace locations to CHCs. Our results indicate that CHCs have the potential to maintain current accessibility levels and even enhance them in the years to come. Moreover, we demonstrate that even redistributing current capacity can improve future accessibility.

Poverty has become a serious development problem, including in West Papua Province. This research has become a fundamental issue because West Papua tends to be difficult to get out of poverty problems, which has the second-highest percentage of poverty in Indonesia. This study aims to analyze the determinants of poverty in West Papua. The dependent variable is poverty, and the independent variables are population density, unemployment, human development index, and the average length of schooling. This research uses multiple regression analysis time series, 11 years period. The finding of this study is that unemployment has a positive and significant effect on poverty. Meanwhile, the human development index and the average length of schooling have a negative and significant effect. The implication is that the human development index and the average length of schooling must be expanded in scope to open access to people living in remote areas. The region's characteristics and the population tend to live in the interior, so accessibility is an important factor in alleviating poverty in West Papua.

Balancing is an essential challenge in healthcare systems that requires effective strategies. This study aims to address this crucial issue by suggesting a practical approach. We show the potential of balancing a regional healthcare system to improve its utility. We consider a regional healthcare system comprising multiple hospitals with different sizes, capacities, quality of service, and accessibility. We define a utility function for the system based on the sectorization concept, which endeavors to form a balance between hospitals in terms of essential outputs such as waiting times and demands. The dynamic nature of the system means that this balance degrades over time, necessitating periodic sectorization, which is called resectorization. Our methodology stands out for incorporating resectorization as a dynamic strategy, enabling more flexible and responsive adaptations to continuously changing healthcare needs. Unlike previous studies, based on a system-oriented approach, our resectorization scenarios include the periodic closure of some hospitals. This enables us to enhance both the capacity and quality of healthcare facilities. Furthermore, in contrast to other studies, we investigate the states of diminishing demand throughout the resectorization process. To provide empirical insights, we conduct a simulation using data from a real-world case study. Our analysis spans multiple time periods, enabling us to dynamically quantify the utility of the healthcare system. The numerical findings demonstrate that substantial utility improvements are attainable through the defined scenarios. The study suggests a practical solution to the critical challenge of balancing issues in regional healthcare systems.
