Geographic Disparities in Access to Syringe Services Programs Among Young Persons With Hepatitis C Virus Infection in the United States

Abstract: Using commercial laboratory data, we found 80% of 29382 young persons currently infected with hepatitis C virus lived >10 miles from a syringe services program. The median distance was 37 miles, with greater distances in rural areas and Southern and Midwestern states. Strategies to improve access to preventive services are warranted.

“…The USA, Brazil and Canada share the challenge of providing equitable access to health across extensive, varied geographical regions, with rural populations often living long distances from heath care services. In the USA, this creates a particular problem in tackling the rural opioid epidemic, with an estimated 80% of all HCV-infected people aged less than 30 years living more than 10 miles from a syringe service program 258 . This situation underscores the importance of combating the rural opioid epidemic using diverse strategies, including integration of HCV testing and treatment services into syringe services programs, and designating pharmacies as sources of safe injection equipment.…”

“…This high cost is due, in part, to the success of other harm reduction strategies suppressing the rate of HIV transmission. However, many PWID in the rural areas at risk for HIV outbreaks do not have access to other prevention efforts, such as SSPs (Canary et al, 2017; Des Jarlais et al, 2015). In these settings, PrEP may be the only option for preventing HIV among many PWID.…”

“…Without clear evidence of success, NEP implementation expanded in an attempt to curtail the spread of HCV. To date, there has been no direct evidence demonstrating that NEPs reduce HCV transmission in the United States . In fact, there are no randomized control trials directly evaluating the effectiveness of NEPs and HCV transmission.…”
