Introduction: Emergency departments (ED) are unpredictable and prone to diagnostic errors. In addition, non-emergency specialists often provide emergency care in Japan due to a lack of certified emergency specialists, making diagnostic errors and associated medical malpractice more likely. While several studies have investigated the medical malpractice related to diagnostic errors in EDs, only a few have focused on the conditions in Japan. This study examines diagnostic error-related medical malpractice lawsuits in Japanese EDs to understand how various factors contribute to diagnostic errors. Methods: We retrospectively examined data on medical lawsuits from 1961-2017 to identify types of diagnostic errors and initial and final diagnoses from non-trauma and trauma cases. Results: We evaluated 108 cases, of which 74 (68.5%) were diagnostic error cases. Twenty-eight of the diagnostic errors were trauma-related (37.8%). In 86.5% of these diagnostic error cases, the relevant errors were categorized as either missed or diagnosed incorrectly; the others were attributable to diagnostic delay. Cognitive factors (including faulty perception, cognitive biases, and failed heuristics) were associated with 91.7% of errors. Intracranial hemorrhage was the most common final diagnosis of trauma-related errors (42.9%), and the most common initial diagnoses of non-trauma-related errors were upper respiratory tract infection (21.7%), non-bleeding digestive tract disease (15.2%), and primary headache (10.9%). Conclusion: In this study, the first to examine medical malpractice errors in Japanese EDs, we found that such claims are often developed from initial diagnoses of common diseases, such as upper respiratory tract infection, non-hemorrhagic gastrointestinal diseases, and headaches.

An alive publication is a new genre for presenting the results of scientific research, where the scientific work is published online, and then is constantly being developed and improved by its author. Serious errors and typos are no longer fatal, nor do they haunt the author for the rest of his or her life. The reader of an alive publication knows that the author is constantly monitoring changes occurring in this branch of science. Alive publication faces the inertia of scientific publishing traditions and, in particular, traditional bibliometrics. Unfortunately, at present, the author who supports an alive publication is dramatically losing out on many generally accepted bibliometric indicators. The alive publication encourages the development of the bibliography apparatus. Each bibliographic reference will soon have to contain on-the-fly attributes such as attendance, number of external links, date of the last revision, etc. In the opinion of the writer of these lines, as the alive publication spreads over to the scientific world, the author’s concern for the publication’s evolution will become like a parent’s care for the development of a child. The Internet will be filled with scientific publications that do not lose their relevance with time.

3D Scanning/Printing: A Technological Stride in Sculpture

The creation of innovative tools, objects and artifacts that introduce abstract ideas in the real world is a necessary step for the evolution process and characterize the creative capacity of civilization. Sculpture is based on the available technology for its creation process and is strongly related to the level of technological sophistication of each era. This paper analyzes the evolution of basic sculpture techniques (carving, lost-wax casting and 3D scanning/printing), and their importance as a culture footprint. It also presents and evaluates the added creative capacities of each technological step and the different methods of 3D scanning/printing concerning sculpture. It is also an attempt to define the term “material poetics”, which is connected to sculpture artifacts. We conclude that 3D scanning/printing is an important sign of civilization, although artifacts lose a part of material poetics with additive manufacturing. Subsequently, there are various causes of the destruction of sculptures, leaving a hole in the history of art. Finally, this paper showcases the importance of 3D scanning/printing in salvaging cultural heritage, as it has radically altered the way we “backup” objects.
