Genotypic Susceptibility Scores and HIV Type 1 RNA Responses in Treatment-Experienced Subjects with HIV Type 1 Infection

Abstract: This study compared the role of genotypic susceptibility scores (GSS) as a predictor of virologic response in a group (n = 234) of HIV-infected, protease inhibitor (PI)-experienced subjects. Two scoring methods [discrete genotypic susceptibility score (dGSS) and continuous genotypic susceptibility score (cGSS)] were developed. Each drug in the subject's regimen was given a binary susceptibility score using Stanford inferred drug resistance scores to calculate the dGSS. In contrast to the dGSS, the cGSS model w… Show more

“…Although some studies reported that GSS had no impact on viral responses [Cingolani et al, 2002;De Luca et al, 2003;Mazzotta et al, 2003], most investigations support genotypic or phenotypic summary measures of regimen potency as independent predictors of viral response [DeGruttola et al, 2000;Haupts et al, 2003;Jiang et al, 2003;Loutfy et al, 2004;Badri et al, 2007;Anderson et al, 2008].…”

“…Currently, none of the summary measures of ARV regimen potency have been fully standardized or validated. The genotypic sensitivity score (GSS), partly because of its more consistent associations with virologic benefit, its preservation of future treatment options, and the intuitive nature of the metric, has been used more commonly [DeGruttola et al, 2000;Haupts et al, 2003;Torti et al, 2003;Ziermann et al, 2004;Badri et al, 2007;King et al, 2007;Anderson et al, 2008]. Few investigations of the immune or clinical disease benefits of GSS in HAART-treated patients have been conducted.…”

“…1). The genotypic susceptibility score (GSS) and phenotypic susceptibility score (PSS) of the background antiretroviral regimen after optimization at week 2 were 0.37 and 1.17, respectively (2,55). VCV was discontinued due to a lack of virologic response 2 days after the week 28 blood draw (defined as 28 weeks from the start of VCV therapy), a time point notable for maximal VCV resistance and a maximal percent inhibition (MPI) of 0%.…”

“…stanford.edu). Genotypic susceptibility scores (GSS) were calculated based on mutation scores obtained from this database with modifications described by Anderson et al [2008]. HIV-1 subtypes were characterized using the phylogenetic method using reference sequences from the HIV Sequence Compendium 2009 as described previously [Parczewski et al, 2010].…”
