Alcaligenes faecalis, an emerging potential pathogen, is commonly found in soil, water, and hospital environments. It is usually a harmless saprophyte in the microbiota of the human gut but occasionally may give rise to infections especially in immunocompromised hosts. There are reports of A. faecalis infections in patients with acute or chronic otitis media (1), urinary tract infections (UTI) (1,2), wounds (3), skin and soft tissue infections (4), endophthalmitis (5), pancreatic necrosis, and abscess (6). These are usually opportunistic infections and are often linked to contamination of hospital equipment, such as nebulizers and respirators, or fluids used for lavage (2). Filipe and coworkers (7) reported in Angola the isolation of A. faecalis from patients with chronic suppurative otitis media due to the use of bird droppings as a traditional remedy to prevent ear discharge. A. faecalis infection is often difficult to treat due to increased resistance to many antimicrobials, such as β-lactams, aminoglycosides, and quinolones. Previous studies have revealed the presence of extended-spectrum β-lactamase (ESBL)-producing A. faecalis in clinical samples. ESBLs are a group of enzymes capable of breaking down various β-lactam antibiotics. ESBL types PER-1 and TEM-21 have been detected in urinary samples from intensive care patients in Northern Italy

Background: Alcaligenes faecalis is usually opportunistic infection in humans. Alcaligenes faecalis infection is often challenging to treat due to its increased resistance to many antibiotics. The results from a clinical study of Alcaligenes faecalis infection may help improve patients’ clinical care. Methods: We conducted a retrospective analysis of all patients presenting with Alcaligenes faecalis infection from January 2014 through December 2019. The medical records of all patients were reviewed for demographic information, clinical symptoms and signs, comorbidities, use of intravenous antibiotics within the past 3 months, bacterial culture, antibiotics sensitivity test, and clinical outcomes.Results: Sixty-one cases of Alcaligenes faecalis infection were seen during the study period, including 25 cases of cystitis, 9 cases of diabetic foot ulcer with infection, 8 cases of pneumonia, 7 cases of acute pyelonephritis, 3 cases of bacteremia, 9 cases of infection at specific sites. Thirty-seven patients (60.7%) had a history of receiving intravenous antibiotics within 3 months of the diagnosis. Fifty-one (83.6%) cases were mixed with other bacterial infections. Extensively drug-resistant infections have been reported since 2018. The highest antibiotic sensitivity rate for Alcaligenes faecalis, 66.7%, was reported in 2019. Ciprofloxacin and piperacillin/tazobactam are not good choices for empiric therapy of Alcaligenes faecalis infection. Conclusions: The most frequent Alcaligenes faecalis infection sites were in the urinary tract, skin and soft tissue, and lung. Alcaligenes faecalis showed markedly decreased sensitivity to commonly used antibiotics and, more recently, extensively drug-resistant Alcaligenes faecalis infections have emerged.

Alcaligenes aquatilis QD168 is a marine, aromatic hydrocarbon-degrading bacterium, isolated from an oil-polluted sediment of Quintero Bay, an industrial-coastal zone that has been chronically impacted by diverse pollutants. The aims of this study were to characterize the phylogenomic positions of Alcaligenes spp. and to characterize the genetic determinants and the physiological response of A. aquatilis QD168 to model environmental stressors (benzene, oxidizing agents, and salt). Phylogenomic analyses, using 35 housekeeping genes, clustered A. aquatilis QD168 with four other strains of Alcaligenes spp. ( A. aquatilis BU33N, A. faecalis JQ135, A. faecalis UBA3227, and A. faecalis UBA7629). Genomic sequence analyses of A. aquatilis QD168 with 25 Alcaligenes spp., using ANIb, indicated that A. aquatilis BU33N is the closest related strain, with 96.8% ANIb similarity. Strain QD168 harbors 95 genes encoding proteins of seven central catabolic pathways, as well as sixteen peripheral catabolic pathways/reactions for aromatic compounds. A. aquatilis QD168 was able to grow on 3-hydroxybenzoate, 4-hydroxybenzoate, benzoate, benzene, 3-hydroxycinnamate, cinnamate, anthranilate, benzamide, 4-aminobenzoate, nicotinate, toluene, biphenyl and tryptophan, as sole carbon or nitrogen source. Benzene degradation was further analyzed by growth, metabolite identification and gene expression analyses. Benzene strongly induced the expression of the genes encoding phenol hydroxylase ( dmpP ) and catechol 1,2-dioxygenase ( catA ). Additionally, 30 genes encoding transcriptional regulators, scavenging enzymes, oxidative damage repair systems and isozymes involved in oxidative stress response were identified. Oxidative stress response of strain QD168 to hydrogen peroxide and paraquat was characterized, demonstrating that A. aquatilis QD168 is notably more resistant to paraquat than to H 2 O 2 . Genetic determinants (47 genes) for osmoprotective responses were identified, correlating with observed high halotolerance by strain QD168. The physiological adaptation of A. aquatilis QD168 to environmental stressors such as pollutants, oxidative stress and salinity may be exploited for bioremediation of oil-polluted saline sites.
