The aim of this study was to investigate the association between the single-nucleotide polymorphisms (SNPs) of the interleukin 22 (IL-22) gene and systemic lupus erythematosus (SLE) in a Chinese population. Three IL-22 SNPs (rs2227485, rs2227513 and rs2227491) were genotyped using SNaPshot SNP genotyping assays and identified by sequencing in 314 SLE patients and 411 healthy controls. The IL-22 level of serum was assessed by enzyme-linked immunosorbent assay (ELISA) kits. Data were analysed by spss version 17.0 software. We found that rs2227513 was associated with an increased risk of SLE [AG versus AA: adjusted odds ratio (aOR) = 2·24, 95% confidence interval (CI) = 1·22-4·12, P = 0·010; G versus· A: adjusted OR = 2·18, 95% CI = 1·20-3·97, P = 0·011]. Further analysis in patients with SLE showed that the AG genotype and G allele were associated with an increased risk of renal disorder in SLE (G versus A: aOR = 3·09, 95% CI = 1·30-7·33, P = 0·011; AG versus· AA: aOR = 3·25, 95% CI = 1·35-7·85, P = 0·009). In addition, the concentration of IL-22 was significantly lower in the rs2227513 AG genotype compared with AA genotype (P = 0·028). These results suggest that rs2227513 polymorphism might contribute to SLE susceptibility, probably by decreasing the expression of IL-22.

Systemic Lupus Erythematosus (SLE) is one of autoimmune disorders. It is thought that the deregulation in the inflammatory markers is due to problem in Forkhead box family member (FOXP3) which is involved in tolerance mechanism. One cannot ignore the role of cytokine-mediated signaling pathways like IL-22. This study was done in the Lahore. Pakistan. The main objective of the study was to monitor the patients of SLE. The purpose was to check the alliance of FoxP3 and IL-22 gene polymorphism. Sixty samples (n = 60) were collected from different hospitals of Lahore. DNA was extracted from EDTA anticoagulated blood of SLE patients. After DNA extraction, IL-22 and FoxP3 genes were polymerized through PCR and further sequenced through Sanger Sequencing method. The FOXP3 exon 2 and three SNPs in IL-2 i.e. rs2227491, rs2227485 and rs2227513 which were already identified were confirmed by Chromas 2.6. The mutations were checked with the help of Nucleotide Blast. Our observation showed that there are nine mutations in studied genotyped samples. The frequency of mutation was 27.27%. Allele T in rs2227485 and, allele C in rs2227513 and rs2227491 was identified in the study predominantly. These 9 mutations were found in case of IL-22 gene. No mutation was observed in Exon 2 of FOXP3 gene in SLE patients. It is concluded that that there may be any association between IL-22 gene polymorphism and SLE but FOXP3 gene was not tangled in the progression of SLE in Lahore population.

Clinical importance of IL-22 cascade in IBD

IL-22 is a relatively new cytokine that is characterized by several unique biological properties. In the intestines, the effect of IL-22 is restricted mainly to non-lymphoid cells such as epithelial cells. Interestingly, the expression pattern and major cellular source of IL-22 have distinct difference between large and small intestines. IL-22 possesses an ability to constitutively activate STAT3 for promoting epithelial cell regeneration and reinforcing mucosal barrier integrity through stimulating the expression of anti-bacterial peptide and mucins. Of note, IL-22 is characterized as a two-faced cytokine that can play not only protective but also deleterious roles in the intestinal inflammation depending on the cytokine environment such as the expression levels of IL-23, T-bet, and IL-22 binding protein. Most importantly, clinical relevance of IL-22 to inflammatory bowel disease has been well highlighted. Mucosal healing, which represents the current therapeutic goal for IBD, can be induced by IL-22. Indeed, indigo naturalis, which can activate IL-22 pathway through Ahr, has been shown in a clinical trial to exhibit a strong therapeutic effect on ulcerative colitis. Despite the beneficial effect of IL-22, continuous activation of the IL-22 pathway increases the risk of colitis-associated cancer, particularly in patients with an extended history of IBD. This review article discusses how IL-22 regulates colitis, how beneficial versus deleterious effects of IL-22 is determined, and why IL-22 represents a promising target for IBD therapy.
