Genetic Features of the Widespread Plasmid Coding for the Carbapenemase OXA-48

Abstract: Complete sequencing of plasmid pOXA-48a carrying the bla OXA-48 gene from a Klebsiella pneumoniae isolate was performed. Its backbone corresponded to that of an IncL/M-type plasmid, in which the bla OXA-48 gene had been integrated through the acquisition of the Tn1999 composite transposon without any other antibiotic resistance gene. Molecular epidemiology using a collection of international OXA-48 producers revealed the wide diffusion of pOXA-48a or closely related plasmids.

“…All carbapenem-resistant K. pneumoniae isolates were tested for the presence of carbapenemase-encoding genes including blaOXA-48, blaNDM-1, blaKPC, blaIMP, and blaVIM by the multiplex PCR using the primers described previously (20)(21)(22). A bacterial suspension (equal to McFarland 4 turbidity) was prepared in sterile water and boiled for 10 minutes.…”

“…Successful horizontal transfer of OXA-48-encoding plasmids within patients had been reported previously in the UK and France [14,15]. In addition, Berger et al showed that the mobilization of the OXA-48 gene is not restricted to the transfer of a single plasmid, but may also be involved in multiple rearrangements from different plasmids originating from different Enterobacteriaceae species [16].…”

“…The bla OXA-48 gene has been almost always located on a conjugative plasmid of ca. 62 kb assigned to the IncL/M group and has been identified only in enterobacterial species [5,6]. Both CTX-M-15 and OXA-48 have been detected in several enterobacterial species, being particularly frequent in Klebsiella pneumoniae, whilst they are relatively uncommon in E. cloacae [2,3].…”
