The mobile colistin resistance (mcr) gene threatens the efficacy of colistin (COL), a last-line antibiotic used in treating deadly infections. For more than six decades, COL is used in livestock around the globe, including Africa. The use of critically important antimicrobial agents, like COL, is largely unregulated in Africa, and many other factors militate against effective antimicrobial stewardship in the continent. Currently, ten mcr genes (mcr-1 to mcr-10) have been described. In Africa, mcr-1, mcr-2, mcr-3, mcr-5, mcr-8, and mcr-9 have been detected in isolates from humans, animals, foods of animal origin, and the environment. These genes are harboured by Escherichia coli, Klebsiella, Salmonella, Citrobacter, Enterobacter, Pseudomonas, Aeromonas, Alcaligenes, and Acinetobacter baumannii isolates. Different conjugative and nonconjugative plasmids form the backbone for mcr in these isolates; however, mcr-1 and mcr-3 have also been integrated into the chromosome of some African strains. Insertion sequences (ISs) (especially ISApl1), either located upstream or downstream of mcr, class 1 integrons, and transposons, are drivers of mcr in Africa. Genes coding multi/extensive drug resistance and virulence are colocated with mcr on plasmids in African strains. Transmission of mcr to/among African strains is nonclonal. Contact with mcr-habouring reservoirs, the consumption of contaminated foods of animal/plant origin or fluid, animal-/plant-based food trade and travel serve as exportation, importation, and transmission routes of mcr gene-containing bacteria in Africa. Herein, the current status of plasmid-mediated COL resistance in humans, food-producing animals, foods of animal origin, and environment in Africa is discussed.

Detection of Clones B2-ST131-C2 and A-ST617 in Escherichia coli Producing Both CTX-M-15 and CTX-M-27 from Tunisian Community Patients

During a two-month period (2017–2018), 336 urine samples positive for Escherichia coli were collected from Tunisian patients. Of the 336 samples, 266 were collected from community patients and 70 from hospital settings. In all, 15 ESBL producers were identified (8 and 7, respectively) and assigned to 13 pulsotypes, including four ESBL-producing E. coli (ESBL-E) with E1 and E2 profiles (2 isolates each) from community patients. The two strains E1 were identified as B2-ST131 subclade C2 and the two isolates E2, A-ST617. The four strains carrying both CTX-M-15 and CTX-M-27, exhibited the multireplicon IncFII/F1A/F1B with the same formula F31:A4:B1. Two isolates with patterns E3 and E4 (Dice coefficient, 78.7%) isolated from community and hospital settings of two geographic areas were assigned to the emerging ST131 C1-M27 subclade and contained the replicon F1:A-:B20. The remaining ESBL-E divided into different sequence types/associated CTX-M: 2 ST131-C2/CTX-M-15 and ST744/CTX-M-55, ST617/CTM-15, ST2973/CTX-M-55, ST6448/CTX-M-15, ST224/CTX-M-15, ST1431/CTX-M-15, and ST38/CTX-M-27, one isolate each. Our study reports for the first time the presence in the Tunisian community of two clones of E. coli, including the virulent clone ST131-C2 harboring both CTX-M-15 and CTX-M-27, and confirms the spread of the emergent clone ST131-C1-M-27, notably in community urinary tract infections.

The large discharges of wastewater in different natural receiving environments, particularly the marine environment, have massively contributed to pollution. This study examined the physicochemical and microbiological quality of wastewater discharged along the Mediterranean Sea. This investigation is one of the few works carried out by scientists along with the Mediterranean countries. Wastewater sampling was carried out in Egypt, Morocco, Algeria, Tunisia, and Italy. Analyses confirmed that the quality of treated wastewater was always better than the ones registered as raw wastewater. In addition, the quality of the treated wastewater and seawater appeared to be adequate and satisfied the standard and recommended limits in vigor in all these Mediterranean countries. These results showed that the wastewater treatment process used in Italy allowed a higher reduction in most tested parameters (COD: 80.44%, BOD5: 58.9%, OM: 70.74%, TOC: 78.4%). The bacteriological quality of these waters and their diversification was assessed by the denaturing gradient gel electrophoresis (DGGE) technique. Firmicutes, Gamma-Proteobacteria, and Beta-Proteobacteria in 42% of the DGGE DNA bands, and predominately some lineages affiliated to cultivable and non-cultivable clones of Bacteroides (58%), were found. The examination of antibiotic susceptibility at the level of 18 strains isolated from various arbitrary water samples showed that most of these isolates exhibited resistance to at least one antibiotic family, and 11 isolates exhibited 100% resistance against aztreonam. This result confirms the large antibiotic-resistance spreading and circulation registered these last decades in the natural environment, conditioned by the extensive and non-controlled use of antibiotics for human therapy and animal feeding.
