| Rhinosporidiosis is a non-contagious, sporadic, chronic granulomatous infection of man and large domestic animals, characterised by the production of large polyps with high recurrence rate. Polyps occur mainly in nasal, ocular regions; cutaneous and disseminated forms are relatively rare. Spread of these organisms to other areas in an affected person may involve blood, lymph or auto infection. Rhinosporidiosis has been reported form more than 70 countries and mainly the reports are concentrated from Sri Lanka and Southern India. Surgical removal of the polyp will be the cure and in some cases dapsone has given some cure. From the day of inception of this disease there had been a controversy for causative agent of this condition among eukaryotic protozoan parasite or algae or fungi or prokaryotic blue green algae which finally confirmed as a fungi Rhinosporidium seeberi. Recent works by various researchers shows that R. seeberi belong to the class Mesomycetozoea. Other mystery remains in its habitat and its inability to be cultured on laboratory media. Similarly, laboratory animal infection does not yield classical form of disease as it does in human and large animals. The pathogen is so mysterious that the mechanism behind evasion of immune mechanism is also not clear. Still various aspects of disease such as mode of transmission, mechanism of infection, biology of rhinosporidiosis, immunological response and laboratory culture techniques are unresolved mysteries due to paucity of documented data which demands more exploration. This review illustrates main features of rhinosporidiosis and systematic approaches for the diagnosis and treatment of this controversial pathogen.

INTRODUÇÃOA rinosporidiose, ou Doença de SEEBER, é um processo inflamatório crônico, granulomatoso, que afeta o homem e, rara mente, alguns animais como bovino, eqüi no e muar. Embora tenham sido descritos casos com comprometimento de outros ór gãos, exigindo por vezes intervenções cirúr gicas delicadas (18), é mais comum nas mucosas do nariz e olho, onde provoca le sões polipóides ou vegetantes, sésseis ou pediculatías. Seu agente etiológico é o Rhinosporidium seeberi (Wernicke, 1900, in Bélou, 19C3|9[), um fungo classificado en tre os Phycomycetes (5), embora esta po sição taxonômica não esteja ainda defini da (4). A observação inicial foi feita na Argentina por Malbran, que, em 1892, iden tificou o micro-organismo num polipo n a sal humano, mas foi Seeber (24) quem pri meiro o descreveu.Apesar de sua distribuição cosmopolita, a rinosporidiose é mais freqüente em re giões tropicais, como acontece na Índia e no Ceilão. As fontes de infecção, modo de transmissão, os aspectos imunológicos, os anim ais susceptíveis à inoculação experi m ental e os possíveis meios de cultura des te fungo são capítulos até o momento des conhecidos. MATERIALO m aterial, objeto do presente trabalho, consta de 11 pacientes atendidos e tra ta dos pelo Serviço de Otorrinolaringologia do Hospital Getúlio Vargas, em TeresinaPiauí, e procedentes dos Estados do Piauí e M aranhão (Quadro 1), no período de 13-9-72 a 26-7-74.De acordo com a bibliografia consulta da, a doença incide principalmente em pa cientes jovens e em geral do sexo m as culino (Quadro 2).O paciente mais jovem tinha 6 anos de idade e o mais idoso, 33 (Quadro I). O tam anho das tumorações, considerando-se o diâm etro longitudinal das mesmas, va riou de 0,7 cm a 4,0 cm.
