Objectives: To assess the prevalence, the change and the determinants of change in polypharmacy in a population-based sample.Methods: Baseline (2003-06) and follow-up (2009-12) data from 4679 participants aged between 35 and 75 years (53.5% women, mean age 52.6 ± 10.6 years) from the population of Lausanne, Switzerland.Polypharmacy was defined by the regular use of ≥5 drugs. Four categories of change were defined: never (no polypharmacy at baseline and follow-up), initiating (no polypharmacy at baseline but at follow-up), maintaining or quitting.Results: Polypharmacy increased from 7.7% at baseline to 15.3% at follow-up. Cardiovascular drugs were the most prescribed medicines at baseline and follow up. Gender, age, obesity, smoking, previously diagnosed hypertension or diabetes or dyslipidemia were significantly and independently associated with initiating and maintaining polypharmacy. Conclusion:In a population-based sample, prevalence of polypharmacy doubled over a 5.6 year period.The main determinants of initiating polypharmacy were age, overweight and obesity, smoking status and previous diagnosed cardiovascular risk factors. --Introduction:The increasing number of cardiovascular and/or neurological patients with polypathologic backgrounds has led practitioners to adapt the technical platform. The objective is to evaluate the indications of one-day hospitalization (ODH) in oral surgery set up in 2013 in the Max Fourestier Hospital (Nanterre), a comparative prospective study is carried out in 2014 and 2015. Material and methods: The evaluation criterion is the rate (%) of local or general perioperative complications requiring intervention or full hospitalization of patients at risk for oral surgery with ODH. Results: Sixty-eight interventions were performed, mainly dental extractions, root planning and biopsies. 52% of patients were women. Cardiovascular diseases (36%) were the majority. 20.58% of the included patients had a complication taken up by the practitioner or managed by the day hospitalization service. This rate of complications was statistically significantly different (x 2 = 9.89, P = 5%, NDF = 1) from the control group (3.80%). Discussion: The highest complication rate in polypathologic patients, confirms the indications of ODH. No conversion to full hospitalization was necessary thus this study shows that ODH allows a safe intake taking charge avoiding a full hospitalization of higher cost.

Résumé : Le médecin généraliste est essentiel dans la prise en charge des patients hypertendus, qui comprend la dispensation de conseils hygiéno-diététiques. Notre objectif était d'analyser les caractéristiques des patients et de leur généraliste associées au fait que le praticien n'ait pas donné ces conseils. Cinquante-neuf généralistes franciliens ont répondu à des questionnaires à l'aide de leurs dossiers, et leurs 1192 patients hypertendus traités, de 25 à 79 ans, ont été interrogés par téléphone. Les analyses, utilisant des modèles mixtes à intercept aléatoire, ont porté sur « ne pas avoir donné de conseils » concernant le sel, la perte de poids, la diminution de la consommation d'alcool et l'exercice physique. Les médecins dispensaient moins de conseils aux hypertendus ayant un faible risque cardiovasculaire et un suivi récent avec peu de consultations dans l'année. Au contraire, une organisation de la pratique avec des consultations longues, au tarif de l'assurance maladie, et la participation à la formation médicale continue étaient associées à plus de conseil, ainsi que le fait que le médecin soit lui-même hypertendu. En ciblant de manière privilégiée les patients qui reçoivent moins de conseils, le développement de programmes d'éducation thérapeutique pourrait améliorer cette situation.
