Abstract:Background: Irritable bowel syndrome is a chronic continuous or remittent gastrointestinal illness characterized by frequent unexplained symptoms that include abdominal pain, bloating, and bowel disturbance, in our country, no study has been conducted using the Ispaghula husk and oxyphenonium bromide for the treatment of IBS patients. Objectives: To compare the efficacy of treatment with Ispaghula husk and Oxyphenonium bromide. Methods: Total sixty patients diagnosed clinically as irritable bowel syndrome irrespective of subtype who fulfilled the Rome II criteria were included equally into two groups-Group-I (Ispaghula group) & Group-II (Oxyphenonium group). In Group-I patients were given 30gm of Ispaghula husk at night daily and in Group-II patients were given 5mg of oxyphenonium bromide. After six weeks the clinical parameters of both the groups recorded in the case record forms were taken for analysis. Results: The mean age of the patients in the Group-I were 33.4±11.9 yrs and that of the patients in the Group-II were 31.0±17.5 yrs. Male and female ratio in group I was 14:1 and in group II was 23:7. Most of the patients were recorded in the age group of 26-30 in both groups . Symptom free patients were graded in 16.7% patients in Group-I and in 20% patients in Group-II. No improvement was occurred in 16.7% patients in Group-I and in 10% patients in Group-II. Conclusions: Ispaghula husk shows the better efficacy to improve the symptoms of IBS like abdominal pain or discomfort and sense of well being than Oxyphenonium bromide. Oxephenonium bromide shows the better efficacy to decrease the stool frequency from base line than Ispaghula husk.

Bloating is one of the most common and bothersome symptoms complained by a large proportion of patients. This symptom has been described with various definitions, such as sensation of a distended abdomen or an abdominal tension or even excessive gas in the abdomen, although bloating should probably be defined as the feeling (e.g. a subjective sensation) of increased pressure within the abdomen. It is usually associated with functional gastrointestinal disorders, like irritable bowel syndrome, but when bloating is not part of another functional bowel or gastrointestinal disorder it is included as an independent entity in Rome Ⅲ criteria named functional bloating. In terms of diagnosis, major difficulties are due to the lack of measurable parameters to assess and grade this symptom. In addition, it is still unclear to what extent the individual patient complaint of subjective bloating correlates with the objective evidence of abdominal distension. In fact, despite its clinical, social and economic relevance, bloating lacks a clear pathophysiology explanation, and an effective management endorsement, turning this common symptom into a true challenge for both patients and clinicians. Different theories on bloating etiology call into questions an increased luminal contents (gas, stools, liquid or fat) and/or an impaired abdominal empting and/or an altered intra-abdominal volume displacement (abdomino-phrenic theory) and/ or an increased perception of intestinal stimuli with a subsequent use of empirical treatments (diet modifications, antibiotics and/or probiotics, prokinetic drugs, antispasmodics, gas reducing agents and tricyclic antidepressants). In this review, our aim was to review the latest knowledge on bloating physiopathology and therapeutic options trying to shed lights on those processes where a clinician could intervene to modify disease course. When patients complain of problems, physicians are used to look for some physical or physiological abnormality ruling out infections, inflammatory or cancer. Unfortunately, when we talk about functional disorders we usually cannot observe any defects and it is only possible for us to know of them through the words of our patients. Developing criteria and guidelines is not an easy process, in particular for functional gastrointestinal diseases, when no disease-based biological markers exist. It is difficult to define a medical disorder in the absence of a biological ‘gold standard’. The Rome II classification is based on the assumption that for each disorder there are symptom custers. Symptoms have in common disturbances in sensory and/or motor gastrointestinal function, which sometimes may overlap across anatomic regions. Nevertheless, several studies provide evidence for site-specific syndromes.
