Background. Homebound status is associated with poor health, comorbidity, and mortality and represents a major challenge for health systems. However, its prevalence among people with disabilities in the basic activities of daily living (ADLs) is unknown. Objectives. The objectives were to: (1) examine the prevalence of the homebound status among middle-aged and older adults with disabilities in ADLs, and (2) identify its clinical, functional, and environmental determinants.Methods. This study included 221 community-dwelling subjects, aged ≥50 years, who applied for long-term care services at the Office for Legal Certification of Long-term Care Need of Coruña (Spain). Each subject had a disability in ADLs and was interviewed by a trained examiner in the subject's home. The participants were considered homebound if they remained inside their home during the previous week. Measures. Demographic, clinical, functional, and environmental factors. Multiple logistic regression was used to determine the factors associated with homebound status. Results. The prevalence of homebound status was 39.8%. A multivariate analysis revealed that the presence of architectural barriers at the home entrance (stairs [OR: 6.67, p < 0.001] or a heavy door [OR: 2.83, p = 0.023]), walking ability limitations (OR: 3.26, p = 0.006), and higher age (OR: 1.05, p = 0.04) were associated with homebound status. Conclusions. Homebound status is a highly prevalent problem among middle-aged and older adults with disabilities in ADLs. Architectural factors in the home and walking ability limitations seem to be important predictors, suggesting that health care interventions should target home adaptations and mobility skills as a means to preventing or decreasing homebound status.

RESUMENObjetivo: Determinar la asociación entre depresión y dependencia funcional en pacientes adultos mayores del Centro Geriátrico Naval. Materiales y métodos: Estudio retrospectivo, transversal y analítico de un análisis secundario de base de datos, con un tamaño muestral por conveniencia de 625 adultos mayores. Resultados: En el análisis de regresión lineal de predicción de dependencia funcional según índice de Barthel, se encontró que, por cada punto obtenido en el cuestionario de Yesavage, disminuye en 4,8±1.82 puntos el índice de Barthel y, por 5 puntos, disminuye 29.99±3.62 puntos. En relación con el Índice de Lawton, se encontraron resultados significativos respecto a la edad, sexo, grado militar, rendimiento físico, fuerza de prensión y depresión. Conclusiones: Se evidenció asociación entre depresión y dependencia funcional para actividades básicas e instrumentales de la vida diaria. Asimismo, que a mayor riesgo de depresión, mayor probabilidad de presentar dependencia funcional según el índice de Barthel, independientemente de otras variables.Palabras Claves: Adulto mayor; depresión; discapacidad (Fuente: DeSC BIREME). Moreover, the higher the risk of depression, the higher the chance of developing functional dependence according to the Barthel Index, regardless of other variables.

Although surgical intervention can be a life-saviour strategy for these patients, several complications could be present in postoperative period, 2 as well as limitations for activities of daily living (ADL) and derangement in quality of life (QoL).3 Postoperative recovery is a dynamic process where biological, physiological, functional and psychological components play a pivotal role. A rise in physical independence and gradual return to ADL are global indicator of functional recovery after surgery. For most patients, the evaluation of ADL is only performed during their hospital stay. Traditionally, patients should probe their capacity to walking, eating, bathing and dressing without help. 4Elderly patients are a special increasing subgroup.5 Because of the ageing-related characteristics, this population has a decreased functional capacity even before hospital admission.6 According to literature reports, more than 10% of elderly patients develop a severe postoperative disability.7 Postoperative evaluations of functional capacity and health-related QoL (HRQoL) have been become as an important component of long-term surgical outcomes, particularly in the elderly population. 8-11The problem is more serious in the case of cancer patients because a higher prevalence of malignancies among the elderly subgroup. 12,13Frequently, surgical indication for elderly patients with comorbidities is based on subjective judgement according to the personal experience of surgeon. In addition, a negative attitude regarding elderly patients can be see among some physicians, which lead to patients with acceptable functional status could be not surgically treated as usually is indicated for younger patients. Furthermore, radical surgical interventions are neither performed in elderly population. This is significant because currently the primary tumour and regional lymphatic node can only be removed by radical intervention with surgical margin, which guarantees a higher probability of cure. 14,15This study was aimed to assess the prevalence of disability and healthrelated quality of life, as well as related risk factors among patients operated for gastrointestinal or thoracic cancer.
