Gender as a Moderator Between Having an Anxiety Disorder Diagnosis and Coronary Artery Bypass Grafting Surgery (CABG) Outcomes in Rural Patients

Abstract: Three findings were noteworthy. First, anxiety disorder is prevalent in rural patients who are undergoing a CABG operation. Second, anxiety was a significant independent predictor of both length of hospital stay and nonroutine discharge for patients receiving CABG surgery. Last, having an anxiety disorder diagnosis increased hospital stay for both males and females; however, females seemed to be impacted more than males.

“…The majority of studies analyzed depressive disorders and mood disorders (N = 40) [ 10 , 11 , 15 - 17 , 20 - 26 , 28 , 30 - 34 , 36 - 38 , 40 , 41 , 44 - 48 , 50 , 52 , 54 , 55 , 57 , 59 - 65 ], followed by anxiety disorders (N = 7) [ 26 , 37 , 47 , 50 , 53 , 63 , 64 ] and post-traumatic stress disorder ( PTSD) (N = 3) [ 37 , 42 , 43 ]. Ten studies investigated any mental disorder [ 18 , 19 , 25 , 29 , 35 , 39 , 49 , 51 , 56 , 58 ].…”

“…Ten studies investigated any mental disorder [ 18 , 19 , 25 , 29 , 35 , 39 , 49 , 51 , 56 , 58 ]. Comorbid mental disorders were assessed by database records (N = 12) [ 10 , 16 , 18 , 23 , 25 , 29 , 39 , 40 , 49 , 51 , 53 , 58 ], clinical interviews (N = 9) [ 11 , 21 , 24 , 41 , 52 , 54 , 60 , 62 , 65 ] and screening questionnaires (N = 35) [ 15 , 17 , 19 , 20 , 22 , 24 , 26 , 28 , 30 - 38 , 42 - 48 , 50 , 54 - 57 , 59 , 61 - 65 ]. Health care costs were assessed by database records (N = 36) [ 10 , 11 , 15 , 16 , 18 , 20 , 22 , 24 , 29 , 30 , 32 - 34 , 36 , 39 , 40 , 42 - 45 , 47 - 54 , 56 - 59 …”

“…In the sample of rural patients the prevalence of anxiety disorder was 27%. Anxiety was a significant independent predictor of both length of hospital stay and non routine discharge [29]. In a prospective study on cardiac-related readmission within 6 months of CABG postoperative anxiety was identified as both a univariate risk factor and a multivariate risk factor for CHD and surgery-related readmission both with and without adjustment for covariates [30].…”

“…For example, Keeley and Driscoll (2010) recently found in a large sample of men and women initially free of CHD that, over a 10-year period, gender moderated the effect of depressive symptoms on CHD mortality, and among women, race-ethnicity moderated the effect of non-leisure activity on CHD mortality. In addition, having an anxiety disorder appears to be more associated with longer hospital length of stay in women having coronary artery bypass graft (CABG) than in men (Dao et al, 2012). The ability to identify moderators in the relationship between psychological factors and health outcomes in CHD will allow mental health practitioners to provide interventions more targeted to the populations that will benefit most from them.…”
