Gender-Affirming Hormone Use in Transgender Individuals: Impact on Behavioral Health and Cognition

Abstract: Purpose of Review With increasing numbers of transgender and gender non-binary individuals presenting for care, knowing how to elucidate the mental health and cognitive outcomes of gender-affirming hormone therapy (GAHT) is necessary. This article reviews the present literature covering GAHT effects on mood, behavioral health, and cognition in these individuals and offers research priorities to address knowledge gaps. Recent Findings Although there are some conflicting data, GAHToverwhelmingly seems to have … Show more

“…A previous meta-analysis demonstrated that cognitive reserve was positively associated with cognitive performance among PLWH, with a large effect size (Cohen's d = 0.9) (39). Besides, we found that female was associated with higher odds of HAND, which is consistent with original reports in other populations (40)(41)(42). Sex difference may exist in prevalence, incidence, and cognitive decline trajectories and in certain type of cognitive impairment (such as non-amnesia MCI) (43).…”

“…Surprisingly, no original studies reported the prevalence of HAND in transgender participants, which is highly under-representative in the minority group. However, hormone therapy, the commonly used treatment in transgender group, is associated with poor executive functioning (41,44). Future studies are thus needed to verify the HAND prevalence and the effects of hormone therapy on cognition in transgender living with HIV.…”

“…in regions associated with emotional learning [24,19]. However, longitudinal studies are scarce and a recent large study did not find any differences between TW pre and post CHT [20,10]. Next to univariate analyses, multivariate approaches offer new insights into the similarities and differences between cisgender and transgender individuals [25,26].…”

“…The finding that transmales, who were significantly more likely to have taken steps toward gender affirmation, presented with better mental health may provide further evidence supporting the effectiveness of gender-affirming treatments in ameliorating distress and improving quality of life. 69,70 Further gender disparities were observed among indices of eating-and weight-related pathology. More specifically, similar to the gender differences observed in cisgender samples that largely indicate that females are disproportionately affected by eating disorders, 71 transfemales in the current sample reported significantly greater eating-and weight-related concerns when compared with transmales.…”
