Gemcitabine-induced atrial fibrillation: A hitherto unreported manifestation of drug toxicity

Abstract: SummaryBackground: Gemcitabine is an antimetabolic drug for solid tumors. Altough its pharmacokinetics as well as its side-effects are well known, paroxysmal atrial fibrillation associated to the administration of this drug has not yet been described. Patients and methods:We describe the case of a 78-year-old man with pancreatic adenocarcinoma who presented repeated paroxysmal atrial fibrillation episodes 18-24 hours after every gemcitabine infusion which resolved with antiarrhythmic drugs. This clinical histo… Show more

“…CP is one of the most related chemotherapeutic agents with atrial fibrillation and it can improve the production of thromboxanes by platelets, activate platelet aggregation and induce thrombotic arachidonic acid pathway 23,24 . Dolci et al 20 have classified the chemotherapy induced cardiotoxicity in two patterns: (1) Acute or subacute cardiotoxicity and (2) CP-induced cardiotoxicity is related to excessive oxidative stress and apoptosis 25 .…”

“…Cases of atrial arrhythmia, supraventricular tachycardia, bradycardia, ST-T wave changes, left bundle branch block, acute ischemic events, myocardial infarction, myopathy have been reported during treatment with cisplatin [4,5]. Furthermore, cisplatin is one of the most frequently cytotoxic drugs associated with atrial fibrillation (AF) [6]. According to some authors, direct myocardial toxicity may have role in the development of cisplatin-related AF [7].…”
