Gelatin-Thrombin Matrix: A New and Simple Way to Manage Recurrent Epistaxis in Hematology Units

Abstract: Introduction. In case of thrombopenia and/or thrombopathy, epistaxes are very difficult to manage. Case Series. Two patients, one with a thrombocytopenia, the other with a thrombopathy, were hospitalized because of repeated active epistaxes after failure of packing. Both patients were successfully treated with an application of Surgiflo without side effects and left the hospital without recurrence of epistaxis. Discussion. Being a subject of many studies dealing with epistaxis, Surgiflo is a simple treatment t… Show more

“…Such agents have been applied to control haemorrhage in a various surgeries ranging from adenotonsillectomy, endoscopic sinus surgery and operations on the anterior base of the skull 31,32 . Côté et al 33 and Buiret et al 34 have described application of these agents to control epistaxis. Tisseel ® (Baxter: a thrombin and fibrinogen product combined with fibrin glue) has been shown to reduce the volume of drainage from a wound in superficial and total parotidectomy, and to reduce the incidence of seromas 35 .…”

“…Surgiflo® is a dehydrated gelatin-thrombin matrix, currently used in neurosurgery and in thoracic and vascular surgery [41][42][43] , which recently has been proposed as an alternative to nasal packing after functional endoscopic sinus surgery 41 and in the management of recurrent epistaxis 42 . A recent publication reports some advantages to use it in case of HHT as an alternative to more invasive techniques 43 .…”
