Geisinger high-risk osteoporosis clinic (HiROC): 2013–2015 FLS performance analysis

Abstract: FLS programs use case finding strategies and address secondary fracture prevention. GHS HiROC's performance and initiation of drug therapy in this fracture patient population contrasts with GHS-PCP care's much lower rate of treatment, documenting the need for ongoing FLS care. Importantly, the results of this analysis have prompted the beginnings of GHS programmatic changes, designed to narrow the reported care gaps in this mature FLS.

“…(4)(5)(6)(7)(8)(9)(10)(11) FLS programs are cost-effective or cost-saving in several different practice settings, (8) and have been broadly and successfully adopted internationally. A substantial, growing body of literature demonstrates that the most effective organizational approach to secondary fracture prevention is a multidisciplinary case management approach that frequently takes the form of a fracture liaison service (FLS).…”

“…Another study has shown little difference in adherence at 24 months if an FLS did or did not actively follow up and suggested that longer term adherence should not be a focus for FLSs [69]. However in the realworld setting, recorded persistence of anti-osteoporosis therapy for FLS is sub-optimal and so remains an important component for an FLS [20,70,71]. Further, one of the few randomised trials of FLS, which focused on patients with upper limb fractures in the ambulatory setting, demonstrated that the FLS arm had a lower adherence to anti-osteoporosis medication at 24 months compared with usual care [72].…”
