Gated SPECT in patients with hypertrophic obstructive cardiomyopathy undergoing transcoronary ethanol septal ablation

Abstract: TESA is an effective technique for relieving left ventricular outflow obstruction in patients with HOCM. Myocardial gated SPECT can identify the presence and location of infarction after TESA.

“…Treadmill, bicycle exercise, atrial pacing, dipyridamole, and adenosine were examined in this population. Defects, both reversible and fixed, were noted from 10% to 100% of the patients imaged (see Table 3 from Keng et al [48] for a summary). Most defects involve the septum but can occur in other walls.…”

“…The prevalence of both reversible and fixed perfusion abnormalities is quite common and has been reported to be between 20 and 75% [43][44][45][46][47]. Data from one of the largest series consisting of 102 patients with HCM (mean age 54 AE 16 years, 56% men, 55% obstructive) reported 54% prevalence of perfusion abnormalities [46].…”

“…58 Finally, high prevalence of late gadolinium enhancement, primarily in a patchy, multifocal mid-wall distribution in areas of hypertrophy, has been found by cardiac magnetic resonance imaging 59,60 and has been correlated with the presence of non-sustained ventricular tachycardia 60-63 and cardiac mortality. 64 Despite such convincing evidence in the literature, assessment of myocardial ischemia has not been considered part of the evaluation and clinical management of patients with HCM.…”
