Background: Roux-en-Y reconstruction is a common anastomosis technique during gastrectomy in gastric cancer. There is a lack of studies on gallstones after Roux-en-Y reconstruction gastrectomy. This study investigated the incidence and potential risk factors associated with gallstones after Roux-en-Y reconstructive gastrectomy in gastric cancer. Methods: The study analyzed data from gastric cancer who underwent radical gastrectomy and Roux-en-Y reconstruction at two hospitals between January 2014 and December 2020. The patients fall into distal and total gastrectomy groups based on the extent of gastrectomy. The cumulative event probability curve was plotted using the Kaplan-Meier, and differences in gallstone between groups were evaluated using the Log-Rank. Propensity score matching (PSM) was applied to construct a balanced total versus distal gastrectomies cohort. A Cox regression was employed to analyze the risk factors for gallstones after Roux-en-Y reconstructive gastrectomy in gastric cancer. Further subgroup analysis was performed. Results: 531 patients were included in this study, 201 in the distal gastrectomy group and 330 in the total gastrectomy. During the follow-up, there were 170 cases (32.02%) of developed gallstones, 145 cases of gallstones constituting 85.29% of all gallstones within two years after gastrectomy. Then, to reduce the impact of bias, a 1: 1 propensity score matching analysis was performed on the two groups of patients. A total of 344 patients were evaluated, with each subgroup comprising 172 patients. In the matched population, the Cox regression analysis revealed that females, BMI ≥ 23 kg/m2, total gastrectomy, No.12 lymph node dissection, and adjuvant chemotherapy were risk factors for gallstones after Roux-en-Y reconstructive gastrectomy. Subgroup analysis revealed a significantly higher incidence of gallstones after open total gastrectomy than after open distal gastrectomy. Conclusion: Significantly higher incidence of gallstones within two years after Roux-en-Y reconstructive gastrectomy for gastric cancer. Patients with these risk factors should be followed closely after gastrectomy to avoid symptomatic gallstones.

Esophageal cancer (EC) ranks among the most prevalent malignant tumors affecting the digestive tract. Esophageal squamous cell carcinoma (ESCC) stands as the prevailing pathological subtype, encompassing approximately 90% of all EC patients. In clinical stage II-IVA locally advanced ESCC cases, the primary approach to treatment involves a combination of neoadjuvant therapy and surgical resection. Despite concerted efforts, the long-term outcomes for ESCC patients remain unsatisfactory, with dismal prognoses. However, recent years have witnessed remarkable strides in immunotherapy, particularly in the second- and first-line treatment of advanced or metastatic ESCC, with the development of monoclonal antibodies that inhibit programmed death 1 or programmed death ligand 1 demonstrating encouraging responses and perioperative clinical benefits for various malignancies, including ESCC. This comprehensive review aims to present the current landscape of perioperative immunotherapy for resectable ESCC, focusing specifically on the role of immune checkpoint inhibitors during the perioperative period. Additionally, the review will explore promising biomarkers and offer insights into future prospects.

A colelitíase, também conhecida como cálculos biliares, é uma condição comum que afeta milhões de pessoas em todo o mundo. Epidemiologicamente, é mais prevalente em mulheres, especialmente na faixa etária entre 40 e 60 anos, embora também possa ocorrer em homens e em idades mais jovens. A fisiopatologia da colelitíase está relacionada à supersaturação da bile com colesterol, pigmentos biliares ou ambos, levando à formação de cálculos. Fatores de risco incluem obesidade, dieta rica em gordura e rapidamente digeríveis, gravidez e uso de contraceptivos orais. O diagnóstico da colelitíase geralmente é realizado por meio de ultrassonografia abdominal, que é altamente sensível na detecção de cálculos biliares. Outros exames de imagem, como tomografia computadorizada e colangiopancreatografia por ressonância magnética, podem ser utilizados em casos específicos. Além disso, exames laboratoriais podem revelar elevações nos níveis de bilirrubina e enzimas hepáticas. Na abordagem conservadora da colelitíase, os pacientes assintomáticos podem ser monitorados ao longo do tempo, especialmente se os cálculos forem pequenos e não complicados. Mudanças na dieta, como redução da ingestão de gordura, podem ajudar a prevenir a formação de novos cálculos e aliviar os sintomas. No entanto, quando ocorrem sintomas como dor abdominal intensa, náuseas e vômitos, a cirurgia torna-se necessária. A abordagem cirúrgica padrão para colelitíase é a colecistectomia, que pode ser realizada por laparoscopia ou cirurgia aberta, dependendo da gravidade da condição e das características do paciente. A colecistectomia laparoscópica é preferida devido a menores taxas de complicações e tempo de recuperação mais curto. Durante o procedimento, a vesícula biliar, onde os cálculos geralmente estão localizados, é removida, aliviando os sintomas e prevenindo complicações futuras. Por fim, o tratamento da colelitíase envolve uma abordagem multidisciplinar, que pode variar desde observação conservadora até intervenções cirúrgicas, visando aliviar os sintomas e melhorar a qualidade de vida dos pacientes.
