This lecture is a commentary on diagnosis and management of acute alimentary bleeding, with special reference to the problems of the less common causes. It is based on the experience of treating over 4,000 cases admitted to the two gastroenterological wards at the Central Middlesex Hospital.Acute and chronic peptic ulcer accounted for about 80% of the admissions. The remainder included: hiatus hernia (2-3 %), portal hypertension (2-7%), carcinoma of the stomach (2 5 %), Mallory-Weiss syndrome (3 %), and about 40 different uncommon causes (5 2%), with a residue of acute bleeds of uncertain origin.

“…The majority of the tumours have been in the ileum (Schwartz 1966;Mann and Simpson 1957;Gurnett and Hartigan 1960;Hui, Reza and Busuttil, 1978), but in one case the tumour was situated in the duodenal bulb (Loebel, Jerez and Danzig, 1964). In only one out ofthe 5 previous cases did pre-operative investigations locate the source of haemorrhage-a barium meal examination showed a filling defect in the duodenum.…”

Sur 24 cas de carcinoïdes duodénaux recensés par LoebelO [7] à travers la littérature, il n'y avait aucune hémorragie digestive comme cela a été le cas chez nos trois malades. Les tumeurs carcinoïdes duodénales s'associent fréquemment à la maladie de Von Recklinghausen (50 % des cas), à un syndrome de Zolinger-Ellison (38 % des cas) ou à une néoplasie endocrinienne multiple (28 % des cas) [8] même si cela n'est pas le cas de nos deux premières patientes.…”

Les carcinoïdes digestifs sont des tumeurs neuroendocrines dérivant des cellules de Kultshinsky, sécrétrices d’hormones ou de neurotransmetteurs. Les auteurs rapportent ici, 3 cas de tumeurs carcinoïdes digestives au Sénégal. La première observation est celle de Madame TC, 54 ans qui a présenté des épigastralgies aux AINS. A la fibroscopie oesogastroduodénale (FOGD) il y avait une hernie hiatale de petit volume et un polype sessile du deuxième duodénum (D2) d’1 cm de diamètre. Après exérèse à l’anse diathermique, l’histologie montrait un carcinoïde typique. Madame FD, 47 ans était la deuxième patiente, admise en Juillet 2004 pour des épigastralgies et une dyspnée d’effort. L’examen était normal. La FOGD montrait un polype sessile de 5 mm de diamètre de (D2). L’anatomopathologie objectivait un carcinoïde infiltrant la muqueuse et la sous muqueuse avec à l’immunohistochimie, une prolifération épithéliale chromogranine A positive. La TDM montrait 2 hypodensités arrondies de 14,6 mm et 32,2 mm de diamètres, aux lobes droit et gauche du foie d’allure métastatique. Madame CN, 48 ans, suivie pour une maladie de Biermer depuis 1996, était reçue en octobre 2004 pour découverte endoscopique d’un polypoïde gastrique. L’examen physique était normal et à la FOGD il y avait un aspect en fond d’oeil diffus de la muqueuse gastrique, avec masses polypoïdes sessiles multi-métriques à la grosse tubérosité et à la petite courbure. Après exérèse à la pince chaude, A l’histologie on notait un carcinoïde infiltrant et à l’immunohistochimie, la cytokératine et la chromogranine A positives.
