Objectives: Gastroesophageal reflux disease (GERD) is defined as gastroesophageal reflux causing troublesome symptoms or complications. In this study we reviewed the literature regarding the prevalence of GERD symptoms in infants and children. Methods: Databases of PubMed, EMBASE, and Cochrane were systematically searched from inception to June 26, 2018. English-written studies based on birth cohort, school-based, or general population samples of ≥50 children aged 0 to 21 years were included. Convenience samples were excluded. Results: In total, 3581 unique studies were found, of which 25 studies (11 in infants and 14 in children) were included with data on the prevalence of GERD symptoms comprising a total population of 487,969 children. In infants (0–18 months), GERD symptoms are present in more than a quarter of infants on a daily basis and show a steady decline in frequency with almost complete disappearance of symptoms at the age of 12 months. In children older than 18 months, GERD symptoms show large variation in prevalence between studies (range 0%-38% of study population) and overall, are present in >10% and in 25% on respectively a weekly and monthly basis. Of the risk factors assessed, higher body mass index and the use of alcohol and tobacco were associated with higher GERD symptom prevalence. Conclusions: This systematic review demonstrates that the reported prevalence of GERD symptoms varies considerably, depending on method of data collection and criteria used to define symptoms. Nevertheless, the high reported prevalence rates support better investment of resources and educational campaigns focused on prevention.

Background. Opera singers are at risk to develop gastroesophageal reflux, because of the vocational challenges during singing. The singers are reluctant to be submitted to pH-metry fearing throat injuries. We evaluated the gastroesophageal reflux in vocal opera students using a non-invasive salivary test. Design and setting. A prospective, case-control study was carried out in 30 vocal opera students from the Music Academy of the city Cluj-Napoca, Romania, enrolled in the “canto” section. Methods. For control, 20 medical students and residents were enrolled. Each subject filled in a questionnaire and gave saliva samples for the salivary test Peptest™ (RD Biomed). The statistical analysis was made using program R version 3.2.1 (2015-06-18). Results. According to salivary Peptest, vocal opera students had a higher prevalence of gastro-esophageal reflux than the control group (96.67% versus 30%; 29 from 30 vs 14 from 20), P <0.001. Only 50% (15 of 30) of the vocal opera students reported reflux symptoms. Conclusions. Salivary Peptest confirmed a higher prevalence of gastroesophageal reflux in vocal opera students, although not all of them reported reflux symptoms.

Актуальность. Согласно данным литературы, частота гастроэзофагеальной рефлюксной болезни (ГЭРБ) в популяции составляет от 7 до 60 %, а у детей, по данным различных авторов, колеблется от 2–4 до 8,7–49 %. У детей ГЭРБ часто формируется на фоне хронической гастродуоденальной патологии. Цель. Изучение эндоскопической картины верхних отделов пищеварительного тракта при их сочетанном поражении у детей. Материалы и методы. Под наблюдением находилось 120 детей с сочетанными ГЭРБ и хронической гастродуоденальной патологией. Диагнозы верифицированы с помощью эзофагогастродуоденоскопии. Дети были разделены на группы в зависимости от характера поражения слизистой пищевода. В I группу вошли 35 детей с эрозивным и язвенным эзофагитом; во II группу включены 43 ребенка с катаральным эзофагитом; III группу составили 42 ребенка с ГЭРБ без эзофагита. Результаты. Частота гастроэзофагеального рефлюкса III степени достоверно чаще встречается у детей с ГЭРБ и эрозивно-язвенным эзофагитом. При умеренно выраженной гиперемии и отеке слизистой оболочки желудка у детей достоверно чаще (p < 0,05) формируется ГЭРБ без эзофагита. При ярко выраженных воспалительных изменениях слизистой желудка достоверно чаще (p < 0,05) имеет место гастроэзофагеальный рефлюкс с воспалительным поражением слизистой пищевода и формированием как катарального, так и эрозивного эзофагита. Деструктивные изменения слизистой в пищеводе и в луковице двенадцатиперстной кишки (ДПК) не коррелируют между собой. Выводы. При сочетанных ГЭРБ и хронической гастродуоденальной патологии степень выраженности эзофагита зависит от характера поражения слизистой желудка и ДПК.
