The morbidity and mortality due to preterm births is significant, one of the handicaps of preterm delivery is respiratory distress syndrome (hyaline membrane disease). The timely recognition and referral of these babies from peripheral institutions to tertiary care centers of utmost importance for further management. This study was conducted to highlight the application of this forgotten simple, cheap and sensitive single step gastric aspirate shake test.

Background: Dengue illness is a common devastating epidemic in pediatric population. Fever, Thrombocytopenia, hypovolemic shock and bleeding are the common manifestations. Platelet indices are now increasingly being reported by the haematology labs which has shown promise as predictor of bleeding. We have derived a composite index which has a potential to be a better predictor of bleeding in dengue illness.Objective: Primary objective was to describe the clinic haematological profile in serologically proved dengue infection in children. Composite platelet index (CPLI) was calculated by multiplying platelet count (decimals) with Mean platelet volume. Material and methods:This was a retrospective study with data of children admitted for dengue illness collected between January 2015 and October 2015. We have studied a total of 117 children.Results: Median (IQR) age was 9 (6-13) years. 34.1% of the patients had hypotension, 20% had bleeding manifestation and 2.4% developed haemorrhagic shock. Case fatality rate was 0.8%. 74.5% had thrombocytopenia. Both platelet and white cell counts were low in 30.9% of our patients. Median (IQR) CPLI in our patients was 13.5 (6.6-23.1) and in those with mucosal bleed or haemorrhagic shock was 6 (3-9.3). Conclusion:About one quarter of pediatric dengue patients will not have thrombocytopenia at admission. Composite platelet index will be low in patients with mucosal bleeding or haemorrhagic shock.

Gastric Aspirate Shake Test for Prediction of Hyaline Membrane Disease in Preterm Babies

Introduction: Hyaline Membrane Disease (HMD) remains a major cause of neonatal death and morbidity in infants of less than 37 weeks gestation. HMD is an important cause of RDS in newborns. To evaluate whether the Gastric Aspirate Shake Test in Preterm babies can predict the likelihood of Hyaline Membrane Disease. Methods: The Shake Test for prediction of Hyaline Membrane Disease was evaluated in 81 preterm newborns at NICU and Nursery, Patan Hospital. Over 0.5 ml of gastric aspirate was obtained within 1 hour of birth and mixed with an equal amount of normal saline for 15 seconds; 1 ml of 95% alcohol was then added and the mixture was agitated for the next 15 seconds. Then after 15 minutes, the airliquid interface was examined for bubbles. Results: Among 81 neonates, 46 developed HMD. 56.8% (26/46) of neonates with negative shake test developed HMD which was statistically significant(p<0.05). The sensitivity, specificity, positive predictive value, and negative predictive value were 96.29%, 76.47%, 86.6%, and 92.8% respectively. Conclusion: The gastric aspirate shake test is a rapid, simple, and inexpensive procedure and useful in predicting HMD and facilitating early transfer to the neonatal intensive care unit.
