Gastric Outlet Obstruction as the Initial Presentation of Upper Tract Urothelial Carcinoma

Abstract: GOO is often the first sign of advanced upper gastrointestinal neoplasms. The most common neoplasms associated with GOO include gastric, pancreatic, and biliary tract cancers. Urinary tract urothelial carcinoma has been a rarely documented cause of GOO.

“…Metastatic spread of urothelial carcinoma to the gastrointestinal tract is rare and tends to involve the rectum in bladder cancer (8). Upper gastrointestinal symptoms, such as vomiting and epigastric pain due to duodenal obstruction in our specific case, are rare because extrinsic malignancy of the duodenum in urothelial cancers of the upper urinary tract and bladder is uncommon, with few documented cases (9)(10)(11). Tokunaga et al (9) reported two cases of bladder cancer in which abnormal perirectal tissue was initially identified and classified as stage M0.…”

“…Subsequently, both cases evolved with the development of RF adjacent to the duodenal wall, although less pronounced than in our study. On the other hand, Andersen et al (11) described a case of UC of the renal pelvis with retroperitoneal extension leading to duodenal obstruction. Similarly, Iwamoto et al (10) documented a case in which periduodenal tissue was detected and reported as inflammatory changes unrelated to the primary tumor, with the final diagnosis made postmortem.…”

“…To our knowledge, only 12 cases of MGOO caused by UTUC, including 8 case reports (Table S1) and 4 cases from cohort studies (Table 1 and Fig. S1), have been reported in the literature 4–11 . Based on the analysis of 73 cohort studies, only 4 of 5046 patients with MGOO were caused by UTUC (0.08%) (Table 1 and Fig.…”
