Gastric lymphomas compared with lymph node lymphomas in a population-based registry differ in stage distribution and dissemination patterns but not in patient survival

Abstract: Primary gastric NHL shows a restricted dissemination pattern, which gives support to the MALT lymphoma concept. Although this might explain the superior disease free survival observed for gastric NHL patients, it does not translate into better overall survival for these patients.

“…In an analysis of all primary gastric lymphomas in the CCCW database (disseminated cases included), the clinical relevance of this concept was also illustrated. 17 This study also focused on possible specific patterns of dissemination for extranodal lymphomas other than MALT lymphomas. Although the relapse risk for N-NHL and EN-NHL patients was similar, also with respect to the distribution of local and distant failures, a striking difference between these patients was seen with respect to preferential sites for recurrence.…”

“…10,11,13 Moreover, as shown in our combined Dutch and Austrian series of 106 patients, specific patterns of dissemination seem to be present and characteristic to the primary site of presentation, supporting the notion that expression of special homing receptors and adhesion molecules are involved in the trafficking of MALT specific lymphocytes to MALT-containing organs. 2,6,8,9 Additionally, the driving force of antigendependent lymphoproliferation by chronic antigenic stimulation by either microbial pathogens (e.g. H. pylori, C. psittaci, B. burgdorferi) or auto-antigens (e.g.…”

“…According to our previous reports 4,[27][28][29] at presentation three patterns of lymphoma localization were defined: (1) 'primary nodal' NHL with presentation in lymph node, Waldeyer's ring or spleen (n = 155); (2) 'primary extranodal' NHL with presentation at other sites without or with regional lymph node involvement (n = 91); (3) other NHL with disseminated disease at presentation and no possibility of determining the primary site (n = 30); the latter were called 'extensive'. Both in primary nodal and extranodal disease, bone marrow involvement was accepted in cases with a positive staging biopsy.…”
