Gastric emptying in diabetic patients by the 13C-octanoic acid breath test: role of insulin in gastric motility

Abstract: IDDM patients showed delayed gastric emptying compared with NIDDM patients, and the (13)C octanoic acid breath test is useful for evaluating DM patients with delayed gastric emptying.

“…Diabetic autonomic neuropathy affecting the gastrointestinal system may result in gastroparesis, esophageal dysfunction, diarrhea, fecal incontinence, or constipation. Gastroparesis, or delayed gastric emptying, is common in type 1 diabetes, with prevalence rates from approximately 30 to 50% [4,[112][113][114] and appears to be more prevalent in type 1 than in type 2 diabetes [115]. Symptoms of gastroparesis include nausea, vomiting, anorexia, bloating, early satiety, and wide swings in blood sugar [112].…”

“…Alternatives to scintigraphy are: 13C-octanoic acid breath tests (13C-OBT), magnetic resonance imaging (MRI), gastroduodenal manometry, wireless motility capsule ("SmartPill"), autonomic tests, and single photon emission computed tomography [16,17]. Treatment of gastroparesis may involve prokinetic agents, which could accelerate GE and relieve the gastroparetic symptoms [18].…”
