Gastric choriocarcinoma admixed with an α-fetoprotein-producing adenocarcinoma and separated adenocarcinoma

Abstract: We report a case of gastric choriocarcinoma admixed with an α-fetoprotein (AFP)-producing adenocarcinoma. A 70-year-old man was hospitalized for gastric cancer that was detected during screening by esophagogastroduodenoscopy (EGD). Initial laboratory data showed the increased serum level of AFP and EGD revealed a 5-cm ulcerofungating mass in the greater curvature of the gastric antrum. The patient underwent radical subtotal gastrectomy with D2 lymph node dissection and Billroth Ⅱ gastrojejunostomy. Histopathol… Show more

“…In this case, the prognosis of collision tumor of choriocarcinoma and small cell carcinoma of the stomach is not determined, because the patient died occurring severe acute lung injury. As previously reported, curative resection and adequate chemotherapy have been found to promote long-term survival in gastric choriocarcinoma and small cell carcinoma [5], [8], [16]. Both gastric choriocarcinoma and small cell carcinoma are highly aggressive diseases and often spread to other organs.…”

“…Gastric hepatoid adenocarcinoma or GAED is one of the representative histologic types of AFP-producing gastric cancer. AFP production is generally considered the result of retrodifferentiation of tumor cells into fetal cells capable of producing AFP [1,28]; therefore, this suggests that gastric hepatoid adenocarcinoma and GAED exhibit retrodifferentiation. However, some hepatoid tumors are negative for AFP expression, and do not have the ability to produce AFP.…”

“…AFP-production is generally considered to be the result of retrodifferentiation of tumour cells into fetal cells capable of producing AFP. 1,35 In this study, one CAED case with a hepatoid component was positive for AFP; thus, AFP might be a sensitive marker for CAED with a hepatoid component. However, more cases are required to confirm the clinical usefulness of markers such as AFP in CAED, because the number of CAED cases in the present study was limited.…”

“…Adenocarcinoma with enteroblastic differentiation is frequently accompanied by hepatoid adenocarcinoma, which is one of the representative histological types of AFP‐producing tumours. AFP‐production is generally considered to be the result of retrodifferentiation of tumour cells into fetal cells capable of producing AFP .
