Gastric cancer associated with gastroenterostomy and partial gastrectomy.

Abstract: Gastric surgery as a definitive method of treatment for simple peptic ulcer has been practised on an increasing scale over the last 50 or 60 years. Despite operation complete relief may not result and the patient may present at widely varying periods afterwards with new or recurrent symptoms. Increasing age brings an increased risk of gastric cancer but the onset of this occasional sequel may be obscured by symptoms which may have followed more immediately after operation. The purpose of this paper is to descr… Show more

“…This has varied from 13 years (Pack and Banner, 1958) through 19 years (Graves and Herrington, 1971 ;Saegesser and James, 1972) to 26 years (Pygott and Shah, 1968), compared with 8: years found in the present series. This is partly explained by the inclusion of cases where the growth has appeared within 3 years of operation.…”

“…Most other reports either contain too few cases (Graves and Herrington, 1971) or do not differentiate between the gastric and duodenal site (Lagache and Vankemniel, 1966;Yanishevsky and Klimenkov, 1968). Other series include a high proportion of patients treated by methods other than gastrectomy (Gray and Lofgren, 1949;Larson et al, 1961;Pygott and Shah, 1968).…”

“…This high unresectability rate is probably due to failure on the part of the clinician to suspect carcinoma in these patients. Other reports (Pygott and Shah, 1968;Graves and Herrington, 1971) have stressed the fact that any patient presenting with further dyspeptic symptoms more than 5 years after surgery for peptic ulcer should be suspected of having developed a gastric stump carcinoma and not a stoma1 ulcer, which tends to present within 5 years.…”
