Gangrenous Stomach in Mesentroaxial Volvulus

Abstract: Volvulus refers to a greater than 180°twisting of a hollow organ about its mesentery and results in luminal obstruction impaired venous return and eventually ischemia though much less common than volvulus of caecum and sigmoid colon ischemia. Gastric volvulus are clinical problems, when not recognized promptly, can lead to necrosis of the involved organ with resultant high morbidity and mortality.

“…Meena et al reported a similar case of mesentroaxial volvulus with gangrene of stomach sparing the antrum. They performed an esophagogastric anastomosis with pyloric dilatation, thus preventing bile reflux and preserving some reservoir function of the stomach [19]. Jethwani et al reported a case of strangulated gastric volvulus with diaphragmatic eventration in a young girl.…”

“…Borchardt's triad) . In acute mesenteroaxial volvulus, these symptoms are unusual: the gastroesophageal junction remains open, nasogastric tube placement is frequently possible, and symptoms may be mild or only develop as complications , as occurred in our patient.…”

“…Surgical treatment options for gastric volvulus include gastropexy and subtotal or total gastrectomy. 8 Subtotal gastrectomy or total gastrectomy followed by gastrojejunostomy or esophagojejunal anastomosis are usually performed in the presence of extensive gangrene. In the present case, the stomach was not extensively gangrenous to consider either subtotal or total resection, as the fundus and antral parts were healthy and vascular.…”
