We have evaluated the long-term outcome of excision, aspiration and no treatment of dorsal wrist ganglia prospectively in 236 (83%) of 283 patients who responded to a postal questionnaire at a mean of 70 months. The resolution of symptoms was similar between the treatment groups (p>0.3). Pain and unsightliness improved in all three treatment groups. The prevalence of weakness and stiffness altered only slightly in all three treatment groups. More patients with a recurrent, or persistent ganglion complained of pain, stiffness and unsightliness (p<0.0001). Patient satisfaction was higher after surgical excision (p<0.0001), even if the ganglion recurred. Twenty-three of 55 (58%) untreated ganglia resolved spontaneously. The recurrence rate was 58% (45/78) and 39% (40/103) following aspiration and excision, respectively. Eight out of 103 patients had complications following surgery. In this study, neither excision nor aspiration provided significant long-term benefit over no treatment.

Ganglion is commonly encountered problem across all age groups. It is one of the most common soft tissue tumors. Most commonly, patients seek treatment for cosmetic reason. Though there are variety of treatment options available, but very few are effective and cosmetically acceptable. Treatments like aspiration alone or in conjunction with hyaloronic acid or triamcinalone have higher recurrence rates. Hitting with book or bible is of historical significance. Surgical excision is best option available, but, leaves scar behind. In these circumstances, treatment by transfixation by no 1 mersilk suture under local anaesthesia on outpatient basis. This is effective as well as cosmetically acceptable method of treatment. In our series of 32 patients, which were treated by transfixation suture, had minimal complication. Silk used in this method helps in making track for escape of ganglion contents following initial procedure. And retaining suture lets the contents escape out and it produces fibrosis within ganglion lining. Thus reducing the chances of recurrences, compared to plain needling. As we call it "needling" is an excellent method with low recurrence, comparable to surgical excision with success rate of 90%.

High-resolution ultrasonography was performed on 14 patients with soft tissue swelling of the wrist and hand. The examination was done using a real-time 10-MHz sector probe placed directly on the lesion. The sonographic image could always demonstrate the liquid nature of cystic lesions. In recent ganglia, the fluid content was anechoic, and the cystic walls were rather thin; in some older lesions, irregular internal echoes and thicker walls were found. Rather often, multiple ganglia were observed, grouped in clusters. The presence of a liquid-filled duct directed to the articular space was demonstrated in 8/11 cases (73%). This finding is considered a diagnostic characteristic of ganglia, and it is very useful for correct surgical planning. In extensor cystic tenosynovitis, the echographic picture was quite different: the anechoic cavity was longitudinal in shape, following the tendon sheath, and the communicating duct was absent. Finally, the sonographic pattern of one case of de Quervain's disease was analyzed and discussed.
