Gamma knife surgery for atypical meningiomas

Abstract: After early tumor shrinkage, high recurrence rates were demonstrated both at the treatment margin and at distant locations in cases treated for AM. There was only one recurrence within the GKS radiation field. For small- and medium-sized AMs GKS may be a safe adjunct to other treatment modalities.

“…4,6 Recently, radiosurgery has been shown to be a safe and effective treatment for the growth of atypical and malignant meningiomas, especially for small lesions. 8,9,20 Harris and colleagues 8 reported the efficacy of early boost radiosurgery following adjuvant radiotherapy for malignant meningioma. The optimal dose of GKS required to control tumor progression depends on several factors, including tumor size, tumor location, dosimetry of prior radiation, and safe radiation dose to the surrounding tissue.…”

“…Although we treated the tumors with relatively small vol ume using higher margin doses, the LC rates were worse than those in the past reports. 3,5,6,18 The favorable out comes in the past literature might be partly attributable to the variability of the treated tumors because of vagueness in histological definition of atypical meningioma before the year of 2000. 5,6,10 In addition, in recently published cases, 3,18 a larger portion of the patients underwent prior radiotherapy, which may also be one of the factors leading to the better LC rates in previous reports than in our study.…”

“…In previous reports, 3,5,6,18 the 5year LC rates after GKS for atypical meningioma were highly variable, ranging from 44% to 83% (Table 3). In contrast, the LC rates in the present study were 74% at 1 year, 39% at 2 years, and 16% at 5 years.…”
