Gallstone ileus with impaction at the neck of a Meckel's diverticulum.

Abstract: We report a case of gallstone ileus in which the stone impacted at the neck of a Meckel's diverticulum. CT demonstrated the gallstone as a calcified mass in the lower abdomen. Gallstone ileus was diagnosed although a more accurate diagnosis was not obtained pre-operatively. The site of impaction was not typical and a blind loop filled with contrast materials was evident. We believe that this is the first report demonstrating this rare condition with imaging.

“…In rare cases enteroliths, can form within a MD and cause an obstruction from impaction within the small bowel. Stasis in the diverticulum in combination with the alkalotic environment of the small bowel can promote precipitation of calcium and is thought to contribute to formation of an MD enterolith[7]. In our case the patient had an impaction of a gallstone in the MD.…”

“…Gallstone ileus is an uncommon cause of obstructions in the general population but is responsible for 25% of mechanical bowel obstructions in the elderly[4-6]. We found only 3 other case reports with mention of gallstone impaction at MD[7-9]. …”

“…Obstruction can be caused by trapping of a bowel loop by a mesodiverticular band, a volvulus of the diverticulum around a mesodiverticular band, and intussusception, as well as by an extension into a hernia sac (Littre's hernia). 1,2 Unusual cases include mechanical obstruction related to peritoneal or mesodiverticular band, 12 long Meckel's diverticulum encircling small bowel loops, 13,14 gall stones at neck of diverticulum, 15,16 axial torsion of diverticulum around its narrow base. 17 Similarly, as in our case, obstruction can be caused by trapping of a bowel loop by a mesodiverticular band.…”

“…Owing to its narrow intraluminal diameter, the terminal ileum is the most frequent site of impaction, but the jejunum, colon and even a Meckel's diverticulum can be involved 10 11. Previous literature has asserted that a gallstone must be larger than 2.5 cm to precipitate obstruction, due to the diameter of the small bowel and the ileocaecal valve 6 12.…”
